摘要
目的观察胆囊切除术与保胆手术在胆囊结石中的临床应用效果及优缺点。方法选择我院收治的110例胆囊结石患者,随机分为对照组和观察组,每组55例。对照组采用腹腔镜胆囊切除术治疗,观察组采用腹腔镜联合纤维胆道镜保胆取石手术治疗。观察比较两组患者的手术时间、术后排气时间、住院时间及并发症发生情况;术后随访两组患者有无腹泻、腹痛、黄疸、胆总管结石等症状出现。结果观察组患者的手术时间、住院时间、术后排气时间及胃肠功能恢复时间均明显短于对照组(P<0.05)。观察组患者并发症发生率为3.64%,低于对照组的18.18%(P<0.05)。随访中,两组的术后腹痛、胆总管结石、黄疸发生率无明显差异(P>0.05),但观察组腹泻发生率显著低于对照组(P<0.05)。结论采用腹腔镜联合纤维胆道镜保胆取石手术治疗胆囊结石,临床效果显著,安全性好,值得推广应用。
Objective To observe the application effect and safety of cholecystectomy and gallbladder preserved surgery in patient with gallstones. Methods One hundred and ten cases of patients with gallbladder stones in our hospital were randomly divided into control group and observation group, with 55 cases in each group. The control group was treated with laparoscopic choledoehoscope, while the observation group received laparoscopic combined fiber choledochoscope confirmed biliary lithotomy surgery. The operation time, postoperative exhaust time, hospitalization time, complications in the two groups were compared, and with or without diarrhea, abdominal pain, jaundice, bravery manager arid other symptoms during the follow-up of the two groups were observed. Results The operation time, postoperative exhaust time, gastrointestinal function recovery time and hospital stay in the observation group were shorter than those in the control group (P〈0.05). The incidence of postoperative complication of the observation group was 3.64%, which was lower than 18.18% of the control group (P〈O.05). There were no significant differences in the incidences of abdominal pain, common bile duct stones, jaundice between the two groups (P〉0.05), but the incidence of diarrhea in the the observation group was less than that in the control group (P〈0.05). Conclusion Laparoscopic combined fiber choledochoscope confirmed biliary lithotomy surgery has good effect and safety on patients with gallbladder stones, which is worthy of popularization and application.
