256层螺旋CT冠状动脉成像联合心电图评估心肌桥与心肌缺血的关系 被引量:5

摘要 目的应用256层螺旋CT冠状动脉成像联合心电图评价心肌桥与心肌缺血的关系。方法对冠状动脉成像发现的131例心肌桥患者资料进行分析,根据有无心肌缺血和临床症状进行分组,比较不同组之间单发心肌桥的特点。结果共有157处心肌桥,其中单发110处,多位于左前降支。心肌缺血组的心肌桥厚度[(1.90±0.08)mm]和壁冠状动脉舒张期横断面面积[(860.15±12.14)mm2]、收缩期横断面面积[(617.46±14.53)mm2]及狭窄率[(23.13±1.76)%]与非心肌缺血组[(1.26±0.14)mm、(945.28±8.49)mm2、(765.38±2.69)mm2及(15.85±1.89)%]比较,差异均有统计学意义(t=8.23、10.46、19.03、7.35,P<0.05)。心肌缺血患者中,有症状者心肌桥厚度[(1.98±0.10)mm]及壁冠状动脉狭窄率[(24.56±0.17)%]大于无症状者[(1.72±0.18)mm、(21.90±0.12)%],差异均有统计学意义(t=3.85、8.49,P<0.05)。结论心肌桥导致的心肌缺血及临床症状的轻重与心肌桥厚度、壁冠状动脉狭窄率有关。 Objective To evaluate the association of myocardial bridge(MB) with myocardial ischemia using 256 slice CT angiography combined with electrocardiogram. Methods The data of 131 patients with MB determined by CT angiography were analyzed. Only the cases with single MB were enrolled and grouped based on myocardial ischemia and clinical symptoms. The MB features were compared between different groups. Results 157 segments of MB were determined in 131 patients, of them, 110 patients had single MB with most located in left anterior descending coronary artery. The thickness of MB, the diastolic and systolic cross-sectional area, and systolic narrowing rate of mural coronary artery were (1.90 ±0.08)mm, (860.15 ±12.14) mm^2, (617.46 ±14.53) mm^2 and (23.13 ±1.76)% in patients with myocardial ischemia and [(1.26 ± 0.14)mm,( 945.28 ± 8.49) mm^2, (765.38 ± 2.69)mm^2 and (15.85 ± 1.59)%], respectively, in patients without myocardial ischemia. The differences between the two groups were significant (all P〈0.05); The thickness of MB and systolic narrowing rate of mural coronary artery were significantly higher in patients with than without ischemic symptoms[(1.98 ±0.10) mm, (24.56 ±0.17)% vs. (1.72 ±0.18)mm, (21.90 ±0.12)%, t=3.85、 8.49, all P〈0.05]. Conclusion The severity of myocardial ischemia and clinical symptoms caused by MB are related to the thickness of MB and systolic narrowing rate of mural coronary artery.
