摘要
目的基于图论的脑网络研究方法探讨脑震荡综合征伴耳鸣患者的脑网络度属性变化及意义。方法选取脑震荡综合征(Post—concussionsyndrome,PCS)患者34例,其中伴有双侧耳鸣17例(PCS伴耳鸣组),不伴有耳鸣(PCS无耳鸣组)17例。另同期招募17例年龄、性别及教育程度相匹配的健康对照者(对照组)。使用基于图论的脑网络研究方法,分析PCS伴耳鸣患者脑网络度属性变化的意义。结果(1)PCS无耳鸣组在左侧眶部额中回(3.13±±1.07)、左侧丘脑(2.51±1.03)、左侧颞上回(3.67±1.31)、右侧前扣带回(3.13±1.25)、右侧后扣带回(2.13±1.08)和右侧缘上回(4.46±1.35)度属性较正常对照组降低(4.41±1.47、3.71±1.08、5.27±2.13、5.51±0.67、5.63±2.16、5.64±1.30)(P〈O.05)。在左侧后扣带回(5.87±1.06)和左侧舌回(4.67±1.48)度属性较正常对照组增高(4.41±1.46、3.21±1.27)(P〈0.05)。(2)PCS伴耳鸣组在左侧后扣带回(3.37±1.54)、左侧海马旁回(3.41±1.62)、左侧杏仁核(2.25±1.43)、左侧角回(4.17±1.02)、左侧颞上回(3.25±1.02)、右侧丘脑(2.35±1.34)、右侧颞横回(3.97±1.62)、右侧颞上回(3.26±1.22)、右侧楔叶(3.18±1.32)和右侧舌叶(3.26±1.42)度属性较正常对照组降低(4.41±1.46、5.27±2.13、3.71±1.08、5.63±2.61、5.64±1.30、3.43±1.33、5.63±2.16、5.13±1.64、5.51±0.67、4.24±0.63)(P〈0.05)。PCS伴耳鸣组在右侧后扣带回(5.76±1.83)、左侧内侧前额叶(6.08±1.62)和右侧楔前叶(6.08±1.06)度属性较正常对照组增高(4.47±1.26、4.41±1.47、4.81±0.62)(P〈0.05)。(3)PCS伴有耳鸣组在左内侧前额叶、左侧杏仁核、左侧海马旁回、右侧颞横回、右侧颞上回、右侧楔叶和右侧舌叶度属性较PCS无耳鸣组降低(P〈0.05)。PCS伴有耳鸣组在右侧后扣带回和左侧岛叶较PCS无耳鸣组增高(P〈0.05)。结论脑震荡综合征伴有耳鸣的患者在脑网络中的相关结构存在度属性的变化,听觉系统、边缘系统和默认网络系统等的相关脑区度属性改变可能是引起PCS耳鸣患者耳鸣的重要因素。
Objective To explorethe changes of degree attribute values and its significance of postconcussion syndrome (PCS) patients with tinnitus by the brain network research method based on graph theory. Methods 34 PCS patients were chosen,including 17 PCS patients with bilateral tinnitus (PCS tinnitus group) and 17 PCS patients without tinnitus (PCS non-tinnitus group). Meanwhile, 17 healthy individuals with the matched age, gender and educational background were recruited as the control. Degree attribute values of PCS patients with tinnitus were figured out with the brain network research method based on graph theory. Results ( 1 ) The degree attribute values of PCS patients without tinnitus at left orbital middle frontal gyrus ( 3.13± 1.07 ), left thalamus ( 2.51 ± 1.03 ), left superior temporal gyrus ( 3.67± 1.31 ), right anterior c in- gulate cortex ( 3.13 ± 1.25 ) , right posterior eingulate cortex ( 2. 13 ± 1.08 ) and right supramarginal gyrus ( 4.46± 1.35 ) were reduced compared with the control group ( 4.41 + 1.47,3.71 ± 1.08,5.27 ± 2.13,5.51 ± 0.67,5.63±2.16 and 5.64+1.30) (P〈0.05). The degree attribute values of PCS patients without tinnitus at left posterior cingulate cortex ( 5.87± 1.06) and left gyrus lingualis ( 4.67± 1.48 ) increased compared with the control group (4.41± 1.46,3.21± 1.27) (P〈O.05). (2) The degree attribute values of PCS patients with tinnitus at left posterior eingulate cortex ( 3.37± 1.54), left parahippoeampal gyrus ( 3.41 ± 1.62 ), left amyg- dala ( 2.25± 1.43 ) , left angular gyrus ( 4.17 ± 1.02 ) , left superior temporal gyrus ( 3.25± l. 02 ) , right thalamus ( 2.35± 1.34 ) , right Heschl gyri ( 3.97 ± 1.62 ) , right superior temporal gyrus ( 3.26 ± 1.22 ) , right euneus ( 3.18± 1.32) and right lingular lobe ( 3.26± 1.42) were decreased, compared with the control group ( 4.41 ± 1.46,5.27+2.13,3.71± 1.08,5.63+2.61,5.64± 1.30,3.43± 1.33,5.63±2.16,5.13+ 1.64,5.51 ±0.67,4.24+ 0.63) (P〈0.05). The degree attribute values of PCS patients with tinnitus at right posterior cingulate cortex (5.76± 1.83) ,left MPFC (6.08± 1.62) and right precuneus (6.08± 1.06) were increased, compared with the control group (4.47±1.26,4.41±1.47,4.81±0.62) (P〈0.05). (3)The degree attribute values of PCS pa- tients with tinnitus at left MPFC, left amygdale, left parahippoeampal gyrus, right Hesehl gyri, right superior temporal gyrus, right cuneus and right lingular lobe were decreased, compared with PCS patients without tin- nitus (P〈0.05). The degree attribute values of PCS patients with tinnitus at right posterior cingulate cortex and left insular lobe increased,compared with PCS patients without tinnitus (P〈0.05). Conclusions PCS patients with tinnitus present the alteration of degree attribute in related brain network structure. The alteration in degree attribute of relevant brain zones in auditory system,limbic system and default network system may be important factors which resuh in tinnitus of PCS patients.
