摘要
目的评价光子治疗仪Carnation-11红光疗法在头颈部肿瘤患者放射治疗引起的放射性皮炎中的应用效果。方法将首次住院治疗头颈部肿瘤的60例患者随机分为实验组和对照组各30例,对照组放疗治疗期间给予常规日常护理(健康宣教、皮肤自我护理、皮肤防护剂等),实验组在放疗治疗期间除常规日常护理,增加光子治疗仪红光疗法,2次,d,10min/次,持续至放疗结束。每天评估患者疼痛及皮肤情况,两组皮肤疼痛及皮炎分度比较。结果所有患者完成放疗,实验组与对照组放射性皮肤炎反应程度比较,统计量U=4.79,P=0.000,差异有统计学意义(P〈0.05),实验组主要以0~2级放射性皮肤炎为主,对照组主要以2-3级放射性皮炎为主。用卡方检验对实验组与对照组口腔疼痛情况进行组间差异比较显示,第2、3、4周的疼痛发生情况,差异有统计学意义(P〈0.05);两组口腔疼痛情况经组内比较,其卡方检验结果分别为(x2=65.083、27.091),均差异有统计学意义(均P〈0.05),说明各组内随着时间延长,疼痛发生情况有所加重。结论光子治疗仪在治疗头颈部肿瘤患者放射治疗中的应用能明显改善患者放射性皮炎,缩短了愈合时间,提高了临床疗效,减轻了患者经济负担和痛苦。
Objective To evaluate the application effect of carnation-11 red light therapy by photon therapeutic apparatus for radiation dermatitis induced by radiation therapy in patients with head and neck cancer. Methods 60 patients with head and neck cancer hospitalized first time were randomly divided into an experimental group and a control group, 30 cases for each group. The control group received routine nursing care during the period of radiotherapy (health education, skin care, skin protective agent, etc.); in addition of routine daily care, the experimental group got red light therapy by photon therapeutic apparatus, twice a day, 10 min per time, until the end of radiotherapy. The pain and skin conditions were evaluated daily. The skin pain and dermatitis degree were compared between these two groups. Results All the patients completed radiotherapy. There was statistical difference in the degree of radiation dermatitis reaction between these two groups ((/=4.79, P=-0.000). The experimental group mainly got 0-2 grade radioactive dermatitis and the control group 2-3.Chi square test showed that there were statistical differences in the incidences of oral pain week 2, 3, and 4 between these two groups (X 2=65.083, 27.091, P 〈 0.05), indicating that the occurrence of pain increased with time. Conclusions Photon therapeutic apparatus can significantly improve the radiation dermatitis in head and neck cancer patients, shorten healing time, increase clinical efficacy, and reduce the patients' economic burden and pain.
