摘要
目的探讨护理干预对心脏瓣膜置换术联合射频消融术疗效以及并发症的影响。方法选取本科进行心脏瓣膜置换术联合射频消融术的患者110例,随机分为观察组和对照组,各55例,对照组给予常规护理,观察组给予综合护理,观察比较2组患者护理后并发症发生率情况、心脏超声变化情况以及护理后手术情况。结果观察组患者的消融时间、主动脉阻断时间、ICU住院时间、体外循环转流时间以及术后住院时间均短于对照组(P<0.05);护理后,2组患者的左心房直径均下降,观察组(38.45±6.12)mm低于对照组(43.56±5.78)mm;护理后,2组患者的EF以及FS均有所上升,观察组(63.45±6.90)%、(38.78±6.78)%高于对照组(53.48±7.02)%、(34.12±6.06)%;护理后,观察组患者出现心律失常、感染、脑栓塞、低心排出量综合征以及肾功能衰竭等并发症发生率均显著低于对照组患者(P<0.05)。结论适当的护理干预能够有效的预防心脏瓣膜置换术联合射频消融术后常见的并发症,改善患者的心脏功能,疗效显著。
Objective To explore the effect of nursing intervention on the efficacy and com- plication of heart valve replacement combined with radiofrequency ablation. Methods A total of 110 patients underwent heart valve replacement combined with radiofrequency ablation were selected and were randomly divided into observation group and control group, with 55 patients in each group. The control group was given routine care, and observation group was given comprehensive care, the inci- dence of complications, cardiac ultrasound changes and nursing after surgery were observed. Results The ablation time, aorta block time, ICU stay, extracorporeal circulation bypass time and postoper- ative hospital stay were significantly shorter in the observation group than in the control group （P 〈 0.05 ）. After nursing, diameter of left atrial in two groups was reduced, but observation group re- duced more than the control group （ P 〈 0.05 ）. After nursing, the EF and FS in observation group were （63. 45 ±6. 90）%, （38. 78 ±6. 78）%, which were significantly lower than （53. 48 ± 7.02） % , （34.12 ± 6.06） % of the control group （ P 〈 0.05）. After nursing, the incidence of com- plications such as arrhythmia, infection, cerebral embolism, low cardiac output syndrome and renal failure in the observation group was significantly lower than that in the control group （ P 〈 0.05 ）. Conclusion Appropriate nursing intervention can effectively prevent complications of heart valve replacement and radiofrequency ablation, and improve the patient＇s cardiac function.
