302例奥氮平不良反应的临床分析 被引量:7

Retrospective Analysis of 302 Cases of Adverse Drug Reactions Induced by Olanzapine

摘要 目的了解并整理奥氮平引起不良反应的规律和特点,为临床医生的合理用药提供参考。方法检索2000~2016年万方数据库、CNKI、维普数据库中收载的奥氮平不良反应文献个案报道55例,收集汇总广州医科大学附属脑科医院和粤北第三人民医院2005~2016年上报的奥氮平不良反应247例,共计302例,进行归类统计分析。结果收集汇总奥氮平引起的不良反应共302例,其中男性171例,明显多于女性131例,19~40 a年龄段患者较多(182例,占60%),肝胆系统损害(24%)和神经系统损害(22%)为常见,主要表现为肝功能异常、锥体外系反应,严重不良反应多发生在肝胆系统和神经系统。结论近年来,奥氮平在治疗精神病中广泛应用,临床使用时应对不良反应密切关注,避免或减少不良反应的发生。 OBJECTIVE To investigate and understand the characters of Olanzapine induced adverse drug reactions (ADR) in order to provide references for rational clinical drug use. METHODS Fifty-five ADR case reports of Olanzapine were extracted from domestic medical journals recorded in China Science Periodical Database ( CSPD), CNKI, and VIP Journals of Chinese Scienc from 2000 to 2016.In addition,247 ADR case reports collected from the Affiliated Brain Hospital of Guangzhou Medical University and The Third People's Hospital of Yuebei between 2005 and 2016 were analyzed retrospectively. RESULTS Among the 302 ADR case reports, 171 cases occurred in male patients and 131 cases occurred in female patients; 182 cases were in patients aged from 19 to 40. The hepatobiliary system and nervous system were the most frequently involved in these case reports (24% and 22%, respectively) and in severe ADR.The most common ADR was abnormal liver function,followed by extrapyramidal reactions. CONCLUSION Clinicians and pharmacists should pay close attention to ADR in clinical use of Olanzapine to reduce the incidence of ADR.
