乌鲁木齐地区妊娠期女性甲状腺激素指标的参考值研究 被引量:9

摘要 目的建立适合于新疆医科大学第一附属医院就诊的妊娠期妇女血清甲状腺激素水平参考值,为乌鲁木齐地区妊娠妇女甲状腺疾病的早期诊断和治疗提供临床依据。方法纳入孕早期妇女1150例、孕中期妇女1221例、孕晚期妇女1449例,共计3820例孕妇及健康非妊娠期妇女1157例。采用电化学发光分析方法检测所有研究对象血清促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、甲状腺球蛋白抗体(Tg-Ab)和甲状腺过氧化物酶抗体(TPO-Ab)的水平,建立妊娠各期甲状腺激素水平参考范围。结果血清FT3、FT4和TSH水平在孕妇各孕期与对照组之间差异均有统计学意义(P<0.01);对照组与妊娠期妇女各孕期之间的TPO-Ab和Tg-Ab水平差异无统计学意义(P>0.05)。结论建立了新疆医科大学第一附属医院妊娠期妇女甲状腺激素参考值范围,有利于对妊娠妇女甲状腺疾病进行筛查。 Objective To establish the reference value range of serum thyroid hormones levels suitable for pregnant women in the First Affiliated Hospital of Xinjiang Medical University so as to provide clinical evidence for early diagnosis and treatment of thyroid disease in pregnant women in Urumqi area. Methods A total of 3 820 pregnant women, including 1 150 early pregnant women, 1 221 mid-pregnancy women and 1 449 late pregnant women, and 1 157 healthy non-pregnant women were included. The electrochemiluminescence method was adopted to detect serum FT3,FT4,TSH,Tg-Ab and TPO-Ab levels in pregnant women and contemporaneous women undergoing healthy physical examination. The reference ranges of the thyroid hormones levels in each stage of pregnancy were established. Results The thyroid hormones reference ranges in pregnant women were obtained. Moreover the serum FT3,FT4 and TSH levels had statistical differences between different pregnancy stages of pregnant women and control group (P〈0.01); but TPO-Ab and Tg-Ab had no statistical differences between different pregnant stages and control group (P〉0.05). Conclusion The reference values ranges of thyroid hormones in pregnant women in our hospital are established, which is helpful to the screening of thyroid diseases in pregnant women and conduces to early diagnosis and treatment.
