摘要
目的:探讨自拟中药汤剂联合复方益母草胶囊治疗剖宫产后恶露不绝患者临床疗效,并观察患者治疗前、后子宫三径变化。方法:截选2014年1月—2016年1月326例于医院行剖宫产分娩产妇进行研究,本次研究对象均出现产后恶露不绝(血热型),其中以复方益母草胶囊治疗者为对照组,5粒/次,2次/d;于对照组治疗基础上采用医院自拟中药汤剂治疗者为治疗组,两组各163例。评价两组患者治疗前、后各项中医证候积分变化,记录两组产后阴道出血时间,统计两组临床疗效,并通过妇科检查与盆底B超检查患者治疗前、后子宫三径情况;记录治疗期间安全性。结果:治疗组治疗后中医证候总积分为(1.63±0.38)分,明显低于对照组(4.51±0.52)分,u=57.09,P=0.00;两组患者治疗后各项中医证候积分明显低于治疗前,差异显著具有统计学意义,P<0.05。治疗组阴道出血时间平均为(9.08±2.38)d,明显短于对照组(11.29±3.02)d,u=7.33,P=0.00;治疗组治疗总有效率92.64%,明显高于对照组81.60%,χ~2=12.25,P=0.00。治疗组治疗后子宫三径和为(14.76±0.57)cm,明显高于对照组(10.33±0.41)cm,u=80.55,P=0.00。两组患者治疗期间未发生明显不良反应。结论:自拟中药汤剂联合复方益母草胶囊治疗剖宫产后恶露不绝疗效显著,且可有利于促进患者临床症状好转,加速其产后子宫复旧,缩短产后恶露干净时间。
Objective： To explore the clinical effect of self-made decoction of Chinese herbal medicine combined with compound motherwort capsule in treatment of cesarean section and observe the changes of the three pathways of the uterus before and after treatment. Methods： In January 2014 to January 2016,326 cases of cesarean section delivery were selected and all subjects were postpartum lochia（ blood heat type）. The control group used the treatment of compound motherwort capsule,5 tablets/time,twice a day. based on the use in the control group,the treatment group used self-made decoction. Two groups had 163 cases in each. The time of vaginal bleeding in two groups was recorded before and after treatment. The clinical curative effect of the two groups was recorded and the three pathways of the uterus before and after treatment were examined by gynecological examination and pelvic floor B-ultrasonography. Safety during treatment was recorded. Results： The total score of TCM syndrome after treatment in the treatment group was（ 1. 63 ± 0. 38） points,significantly lower than the control group＇s（ 4. 51 ± 0. 52）,u = 57. 09,P = 0. 00. Before treatment,the difference was statistically significant（ P〈0. 05）. The average effective rate of vaginal bleeding in the treatment group was（ 9. 08 ±2. 38） d,which was significantly shorter than that of the control group（ 11. 29 ± 3. 02） d,u = 7. 33,P = 0. 00. The total effective rate of the treatment group was 92. 64%,significantly higher than the control group＇s 81. 60%,χ^2= 12. 25,P= 0. 00. The mean diameter of the uterus after treatment was（ 14. 76 ± 0. 57） cm,which was significantly higher than that of the control group（ 10. 33 ± 0. 41） cm,u = 80. 55,P = 0. 00. No significant adverse events were observed during the treatment. Conclusion： Self-made decoction combined with compound motherwort capsule after treatment of cesarean section lochia has no significant effect,and may help to improve clinical symptoms in patients with accelerated postpartum uterine involution and shorten the postpartum lochia clean time.
