摘要
目的探讨能谱计算机X线断层扫描(computed tomography,CT)曲线诊断囊型肝包虫的价值。方法收集2015-09至2016-09囊型肝包虫患者36例,共71枚包虫,经宝石能谱成像(gemstone spectral imaging,GSI)扫描,应用能谱软件GSI General进行分析,测肝包虫与皮下脂肪的最佳对比噪声比(contrast to noise ratio,CNR)为65 keV,在65 keV下测量包虫囊液和囊壁的CT值,得到囊液的能谱曲线图像,计算能谱曲线斜率,对结果进行统计学分析。结果 GSI平扫各型包虫囊液的CT(65 keV)、能谱曲线斜率由小到大变化,各型之间有统计学差异(P<0.05);多子囊型包虫母囊的CT(65 keV)值、能谱曲线斜率与子囊之间有统计学差异(P<0.05);GSI平扫各型包虫囊壁的CT值(65 keV)、能谱曲线斜率由小到大变化,各型之间有统计学差异(P<0.05):单囊型到钙化型包虫的包虫能谱曲线由弓背递增型变为陡然递减型。结论能谱曲线对诊断不同分型肝包虫具有独特优势。
Objective The study objective was to explore the value of the spectral computed tomography (CT) in diagnosing cystic hepatic hydatid. Methods 36 cases of patients with cystic hepatic hydatid were selected from September 2015 to September 2016, which resulted in 71 hydatids. All were scanned by gemstone spectral imaging (GSI) and analysed by GSI General. Contrast to noise ratio (CNR) between hepatic hydatid and subcutaneous fat was 65 keV level. With CNR at 65 keV, the value of CT in hydatid cyst fluid and the capsule wall was measured, and the spectral curve was determined. The slope of the spectral curve was calculated and the results were statistically analyzed. Results GSI scan the CT of hydatid liquid (65 keV) and energy spectrum curve slope changes from small to large, and the differences were statistically significant (P〈0.05). There was significant difference in CT (65 keV) value and the slope of the energy spectrum curve between mother cyst and daughter cyst,and the differences were statistically significant (P〈0.05). The CT values (65 keV) of capsular wall scanned by GSI and the slope of the energy spectrum curve changed from small to large, and the differences were statistically significant (P〈0.05). The energy spectrum curve of the hydatid from single cystic type to the calcification type was changed from increasing camber to precipitous decline. Conclusions Spectral CT of spectral curve has unique advantages in diagnosing different types of hepatic hydatid.
