摘要
目的观察氟哌噻吨美利曲辛联合解郁安神颗粒治疗脑卒中后抑郁症的疗效及对病人血清5-羟色胺(5-HT)和去甲肾上腺素(NE)水平的影响。方法选择缺血性脑卒中抑郁症病人90例,按随机数字表法分为对照组和观察组,每组45例。对照组口服氟哌噻吨美利曲辛,每天早晨服用1片。观察组在对照组基础上给予服用解郁安神颗粒,1袋/次,2次/天。两组连续治疗6周。比较两组汉密尔顿抑郁量表(HAMD)各因子评分和临床疗效。检测两组治疗前及治疗后3、6周血清5-HT和NE水平。结果治疗后,观察组HAMD量表各因子评分显著低于对照组(P<0.01)。观察组总有效率93.33%显著高于对照组75.56%(P<0.05)。治疗后,观察组血清5-HT和NE水平显著高于对照组,差异有统计学意义(P<0.01)。结论氟哌噻吨美利曲辛联合解郁安神颗粒治疗脑卒中后抑郁症疗效显著,其疗效可能与提高病人血清NE、5-HT水平联系密切。
Objective To observe efficacy of flupentixol and melitracen combined with Jieyu Anshen particle in the treatment of depression after cerebral stroke,and the influence on serum levels of 5-hydroxytryptamine(5-HT) and norepinephrine(NE).Methods Ninety patients with depression after cerebral stroke were selected and randomly divided into control group(45 cases) and observation group(45 cases) according to random number table.Patients of control group were orally treated with flupentixol and melitracen,1 tablet per day in morning.Based on control group,cases of observation group were treated with Jieyu Anshen particle,1 bag/time,bid.Two groups were observed for 6 weeks.Scores of HAMD factors and clinical efficacy were compared between both groups.Serum levels of 5-HT and NE were detected in two groups after treatment.Results After treatment,scores of HAMD factors of the observation group were lower than those of control group (P〈0.01).Efficacy in observation group was 93.33% better than that of control group(75.56%)(P〈0.05).After treatment,serum levels of 5-HT and NE were higher than control group with statistical significance after treatment for 3 and 6 weeks (P〈0.01).Conclusion Efficacy of flupentixol and melitracen combined with Jieyu Anshen particle in the treatment of depression after cerebral stroke is significant and may be related with increasing serum levels of 5-HT and NE.
