摘要
目的探讨微血管减压术治疗原发性三叉神经痛的疗效及相关并发症。方法回顾性分析2015年2月至2016年2月解放军第476医院136例经微血管减压术治疗的三叉神经痛患者的临床资料,比较年龄<65岁组与≥65岁组的手术疗效、并发症。结果 136例患者中,<65岁组84例,其中治愈者48例,明显缓解者24例,部分缓解者8例,无效者4例。术后出现面瘫者5例,听力下降3例,脑脊液漏2例,颅内感染1例。≥65岁组52例,其中治愈者36例,明显缓解者8例,部分缓解者4例,无效者4例。术后出现面瘫者2例,听力下降2例,脑脊液漏1例,颅内感染1例,颅内出血4例(幕上出血3例,幕下出血1例)。两者的疗效比较无统计学差异;年老者术后出现颅内出血的风险较高(P<0.05)。结论微血管减压术治疗原发性三叉神经痛效果确切。对于药物控制无效的高龄患者,如无手术禁忌证,可选择手术治疗,术后警惕颅内出血。
Objective To discuss curative effects and related complications of primary pros- opalgia by the treatment of microvacular decompression. Methods Analyze retrospectively clinical data （136 patients with prosopalgia by the treatment of microvacular decompression in the people ＂s liberation army No. 476 hospital） from February 2015 to February 2016,and compare effects of the operation and complications of the group under the age of 65 and the group at or above 65. Re- suits Of the 136 patients,there were 84 patients under the age of 65 years old,of which 48 were cured,24 were obviously relieved,8 were partly relieved and 4 were ineffective. After the opera- tion,facial paralysis occurred to 5 patients, hearing loss to 3, leakage of cerebrospinal fluid to 2 and intracranial infection to 1. There were 52 patients at or above the age of 65,of which 36 were cured,8 were obviously relieved, 4 were partly relieved and 4 were ineffective. After the opera- tion, facial paralysis occurred to 2 patients, hearing loss to 2,leakage of cerebrospinal fluid to 1,in- tracranial infection to 1 and intracranial hemorrhage to 4 （supratentorial hemorrhage to 3 and sub- tentorial hemorrhage to 1）. There was no statistical difference between two groups,while the aged were at a higher risk of postoperative intracranial hemorrhage （P〈0.05）. Conclusion Microvac- ular decompression was effective in treatment of primary prosopalgia. Operative treatment was a- vailable for aged patients who were failed in drug therapy, and more attention should be paid to in- tracranial hemorrhage.
