摘要
目的探究血清鸢尾素(Irisin)与急性冠脉综合征(acute coronary syndrome,ACS)的相关性。方法选取2013年9月至2014年3月于同济大学附属第十人民医院心脏中心住院患者182例,其中男性108例,女性74例;根据入院检测心肌肌钙蛋白T(cTNT)的水平,分为正常对照组(cTNT 0~0.014 ng/ml)、心肌损伤组(cTNT0.015~0.05 ng/ml)、急性冠脉综合征组(cTNT>0.051 ng/ml)。ACS组根据责任血管分为左前降支(left anterior descending branch,LAD)病变组及非左前降支病变组。所有的受试者均进行病史采集、体格检查、血生化指标及血清Irisin水平测定。结果采用单因素方差分析、t检验和Person相关分析进行统计学分析。结果 ACS组的血清Irisin水平较对照明显升高[(145.35±10.11)ng/ml vs(102.68±5.60)ng/ml,P<0.01],左前降支病变组(LAD病变)较非左前降支病变组(非LAD病变)Irisin水平升高[(123.86±8.05)ng/ml vs(108.27±5.72)ng/ml,P<0.05]。Irisin与N-端脑钠肽前体(NT-proBNP)、高敏C反应蛋白(hs-CRP)、空腹血糖(FBG)呈正相关(分别r=0.259,P<0.01;r=0.445,P<0.01;r=0.179,P<0.05)。结论 Irisin在ACS时明显升高,对ACS有预测和诊断意义,有望成为ACS治疗的新型分子靶点。
Objective To investigate the correlation between serum irisin level and acute coronary syndrome (ACS). Methods A total of 182 patients, including 102 males and 74 males, from Shanghai Tenth Peoples Hospital, Tongji University were enrolled in the study. According to the level of cTNT, they were divided into normal control group ( cTNT 〈 0. 014 ng/ml), myocardial injury group ( cTNT 0. 015 - 0. 05 ng/ml) and ACS group (cTNT 〉0. 051 ng/ml). According to the responsible vessels, the ACS group was further divided into left anterior descending branch (LAD) lesion group and no left anterior descending branch lesion group. All the participants received questionnaire survey, physical examination, blood chemistry tests and serum irisin measurement. Univariate analysis, Student's t test and Person correlation were used for data analysis. Results Serum irisin levels were significantly increased in ACS group compared with control [ (145. 35± 10. 11) ng/ml vs (102. 68 ±5. 60) ng/ml, P 〈0. 011. In ACS patients serum Irisin levels were higher in LAD lesions group than those in non-LAD lesion group [ (123. 86 ± 8. 05) ng/ml vs ( 108. 27 ± 5. 72) ng/ml, P 〈 0. 05 3. Irisin was positively correlated with NT-proBNP, high-sensitivity c-reactive protein (hs-CRP) and fasting blood glucose ( FBG ) ( r = 0. 259, P 〈 0. 01 ; r = 0. 445, P 〈 0. 01 ; r = 0. 179,P 〈 0. 05 respectively). Conclusion Serum irisin levels are increased in ACS patients, which might be used as a biomarker in prediction, diagnosis and treatment of ACS.
