孕前体质量指数对妊娠期糖尿病孕妇分娩方式和新生儿体重的影响 被引量:18

Effect of prepregnancy body mass index on delivery modes of pregnant women with gestational diabetes mellitus and neonatal weight

摘要 目的分析孕前体质量指数(BMI)对妊娠期糖尿病(GDM)孕妇分娩方式和新生儿体重的影响,为孕妇体质量管理提供参考依据。方法选取2015年1月-2016年1月于天津市第一医院进行产前检查及顺利分娩的GDM孕妇124例,对孕妇临床资料进行回顾性分析。根据孕妇孕前BMI将其分为体重过低组、体重正常组、体重超重组及肥胖组,比较并分析各组孕妇的分娩方式及新生儿体重。结果肥胖组剖宫产率为81.25%,显著高于体重过低组、体重正常组、体重超重组,差异均有统计学意义(P<0.05)。体重过低组顺产率为43.75%,显著高于体重正常组、体重超重组、肥胖组,差异均有统计学意义(P<0.05)。肥胖组低体重儿发生率明显高于体重过低组、体重正常组、体重超重组,差异均有统计学意义(P<0.05)。肥胖组巨大儿发生率明显高于体重过低组、体重正常组、体重超重组,差异均有统计学意义(P<0.05)。结论 GDM孕妇的分娩方式及新生儿体重均与孕妇孕前BMI有一定关系。孕妇孕前BMI≥28会增加剖宫产率及巨大儿发生率,对新生儿健康产生一定影响。因此,为改善GDM孕妇的妊娠结局,降低母婴并发症的发生率,临床应对孕前BMI过高的孕妇给予积极治疗,将BMI控制在正常范围内。 Objective To analyze the effect of prepregnancy body mass index( BMI) on delivery modes of pregnant women with gestational diabetes mellitus( GDM) and neonatal weight, provide a reference basis for body weight management of pregnant women.Methods A total of 124 pregnant women with GDM were selected from the hospital from January 2015 to January 2016,then the clinical data was analyzed retrospectively. The pregnant women were divided into low weight group,normal weight group,overweight group,and obesity group according to prepregnancy BMI. The delivery modes and neonatal weights in the four groups were compared and analyzed.Results The rate of cesarean section in obesity group was 81. 25%,which was statistically significantly higher than those in the other three groups( P〈0. 05). The vaginal delivery rate in low weight group was 43. 75%,which was statistically significantly higher than those in the other three groups( P〈0. 05). The incidence rate of low weight infants in obesity group was statistically significantly higher than those in the other three groups( P〈0. 05). The incidence rate of macrosomia in obesity group was statistically significantly higher than those in the other three groups( P〈0. 05). Conclusion Delivery modes of pregnant women with GDM and neonatal weight are correlated with prepregnancy BMI to a certain extent. Prepregnancy BMI≥28 will increase the incidence rates of cesarean section and macrosomia,which has a certain impact on neonatal health. Thus,in order to improve pregnancy outcomes of GDM pregnant women and reduce the incidence rate of maternal and infantile complications,the pregnant women with high BMI should be treated actively to control BMI within the normal range.
