摘要
目的初步探讨Angio Jet药物机械偶联血栓清除降低急性下肢深静脉血栓患者下腔静脉滤器置入的可行性。方法回顾性分析2015年2月至2016年8月50例接受血管腔内治疗的急性下肢深静脉血栓患者的相关资料,其中24例患者接受Angio Jet药物机械偶联血栓清除治疗,26例接受经皮导管溶栓(catheterdirected thrombolysis,CDT)治疗,对比2组患者的疗效、并发症及下腔静脉滤器的使用情况。结果 Angio Jet组18例患者血栓完全清除,4例患者血栓大部分清除,2例患者血栓部分清除,CDT组17例患者血栓完全清除,6例患者血栓大部分清除,3例患者血栓部分清除,差异无统计学意义,Angio Jet组尿激酶用量(18±3)万U明显少于CDT组(218±43)万U,Angio Jet组溶栓时间(4.2±0.7)h较CDT组(84.2±5.7)h明显缩短,Angio Jet组并发症发生率17%(4/24)与CDT组15%(4/26)比较差异无统计学意义,Angio Jet组11例(46%)患者置入滤器明显低于CDT组26例(100%)。结论初步研究显示Angio Jet药物机械偶联血栓清除与CDT治疗急性下肢深静脉血栓均有显著疗效,但较CDT治疗降低了下腔静脉滤器的置入。
Objective To explore the feasibility of reducing the placement of inferior vena cava filter in patients with acute deep vein thrombosis by AngioJet rheolytic thrombectomy. Methods Fifty patients with acute lower extremity deep vein thrombosis undergoing catheter-directed interventions from February 2015 and August 2016 were evaluated retrospectively. They were divided into AngioJet group and CDT group, and comparisons were made regards to the treatment outcome between the two groups. Results In the AngioJet group, complete thrombus removal was ac- complished in 18 patients, substantial thrombus removal in 4 patients, and partial thrombus removal in 2 patients, while in the CDT group, complete thrombus removal was accomplished in 17 patients, substantial thrombus removal in 6 patients, and partial thrombus removal in 3 patients (there was no statistical significance). For the two groups, the mean urokinase dose was (0.18±0.03) million unit and (2.18±0.43) million respectively (P〈0.01), the duration of thrombolysis was (4.2±0.7) hours and (84.2±5.7) hours respectively(P〈0.01), the occurrence of complications were 17%(4/24) and 15%(4/26) respectively (there was no statistical significance). The placement of inferior vena eava filter was reduced in AngioJet group 46%(11/24) compared to CDT group 100%(26/26). Conduslon AngioJet rheolytic thrombectomy is a new, safe and effective way for the treatment of acute lower extremity deep vein thrombosis and the placement of inferior vena cava filter was reduced.
