摘要
目的探讨冠状动脉粥样硬化性心脏病(冠心病,CHD)患者血清胱抑素C(Cys-C)、β2微球蛋白(β2-MG)变化与患者左心功能及结构变化的关系。方法选取陕西省人民医院于2015年1月~2016年1月收治100例CHD患者作为CHD组,平均年龄(64.2±11.3)岁,选取年龄、性别基本匹配的100例体检健康人群作为健康组,平均年龄(62.1±12.3)岁,检测两组患者的血清Cys-C、β2-MG及心功能、左心结构指标,包括左室射血分数(LVEF)、室间隔厚度(IVS)、左心室质量(LV mass)、左室舒张末期内径(LVEDd)、左室收缩末期内径(LVESd)、左室舒张末期容积(LVEDv)、左室收缩末期容积(LVESv),并探讨二者间的关系。结果 CHD组患者的Cys-C、β2-MG水平显著高于健康组(P<0.05),CHD组患者IVS、LV mass、LVEDd、LVESd、LVEDv、LVESv水平显著的高于健康组(P<0.05),LVEF水平低于健康组(P<0.05);CHD组患者的Cys-C、β2-MG水平与IVS、LV mass、LVEDd、LVESd、LVEDv、LVESv水平均呈显著的正相关关系(相关系数r分别为0.477、0.403、0.377、0.401、0.455、0.462、0.513、0.536、0.428、0.439、0.527、0.589,P均<0.05),Cys-C、β2-MG与LVEF呈显著的负相关关系(r值分别为-0.544、-0.620,P均<0.05)。结论 CHD患者血清Cys-C、β2-MG水平显著的提高,且与患者的左心结构、功能指标相关。
Objective To investigate the relationship between serum Cystatin C (Cys-C) and β2-microglobulin (β2-MG) in patients with coronary heart disease (CHD) and left ventricular function and structural changes. Methods A total of 100 patients with CHD from January 2015 to January 2015 in Shaanxi province people's hospital were selected as CHD group, mean age (64.2±11.3) years old. 100 healthy people (age and gender matched) as healthy group, with an average age of (62.1±12.3) years old. Serum levels of Cys-C, β2-MG, left ventricular ejection fraction (LVEF), ventricular septal thickness (IVS), left ventricular mass (LV mass), left ventricular end diastolic diameter (LVEDd), left ventricular systolic (LVESd), left ventricular end-diastolic volume (LVEDv), left ventricular end-systolic volume (LVESv) were analyzed. Results The levels of Cys-C and β2-MG in CHD group were significantly higher than those in healthy group (P〈0.05). The levels of IVS, LV mass, LVEDd, LVESd, LVEDv and LVESv in CHD group were significantly higher than those in healthy group (P〈0.05), LVEF level was lower than that in healthy group (P〈0.05). The levels of Cys-C and β2-MG in CHD group were significantly correlated with IVS, LV mass, LVEDd, LVESd, LVEDv and LVESv levels (correlation coefficient r was 0.477,0.403,0.377,0.401,0.455,0.462, 0.513, 0.536, 0.428, 0.439, 0.527, 0.589, P〈0.05), and there was a significantnegative correlation between Cys-C, β2-MG and LVEF (r=-0.544, -0.620, P〈0.05). Conclusion The levels of serum Cys-C and β2-MG in CHD patients were significantly improved, and correlated with the left ventricular structure and function of patients.
