摘要
目的探讨综合治疗干预下维持性血液透析(MHD)高血压患者季节性血压变化的影响因素。方法选择我院血液净化中心2015年1月至2016年6月期间诊治的90例合并高血压的MHD患者为研究对象,均给予综合治疗干预,比较夏季与冬季血压、干体质量、透析间期体质量增加量(IDWG)、尿素清除分数(Kt/V)以及降压药的限定日剂量(DDD),采用Logistic回归分析方法分析夏冬季节血压变化的影响因素。结果冬季患者收缩压、舒张压、干体质量、DDD、IDWG分别为(141.44±10.86)mm Hg、(88.33±6.96)mm Hg、(59.58±4.84)kg、(3.74±1.12)、(2.52±1.15)kg,均明显高于夏季的(137.52±10.43)mm Hg、(84.41±6.25)mm Hg、(59.58±4.84)kg、(3.42±1.03)、(2.11±0.93)kg,差异均有统计学意义(P<0.05);Logistic回归分析结果显示,夏冬季节的干体质量差值是血压变化的独立影响因素(P<0.05)。结论在实施综合治疗干预的前提下,MHD高血压患者仍然存在季节性血压变化,干体质量增加是血压季节变化的独立影响因素。
Objective To explore the influencing factors of seasonal blood pressure in patients with maintenance hemodialysis(MHD) hypertension under comprehensive therapy. Methods A total of 90 cases of MHD patients with hypertension, who admitted to the Blood Purification Center of our hospital from January 2015 to June 2016, were selected and given comprehensive treatment intervention. The dry weight, blood pressure, interdialytic weight gain(IDWG), delivered dialysis dose(Kt/Vurea), defined daily dose(DDD) of antihypertensive drug in summer and winter were compared. Logistic regression analysis was used to analyze the influencing factors of blood pressure during summer and winter. Results The systolic blood pressure and diastolic blood pressure, dry weight, DDD, IDWG in winter season were respectively(141.44 ± 10.86) mm Hg,(88.33 ± 6.96) mm Hg,(59.58 ± 4.84) kg,(3.74 ± 1.12),(2.52 ± 1.15) kg, which were significantly higher than(137.52±10.43) mm Hg,(84.41±6.25) mm Hg,(59.58±4.84) kg,(3.42±1.03),(2.11±0.93) kg in summer(P〈0.05). Logistic regression analysis showed that the difference between summer and winter dry weight is the independent factor affecting the changes of blood pressure(P〈0.05). Conclusion Under the premise of comprehensive treatment intervention, MHD hypertension patients still have seasonal blood pressure changes, and dry weight gain is an independent factor of seasonal changes in blood pressure.
