摘要
目的:探讨超声弹性成像(UE)应变率比值法、改良5分法联合超声乳腺影像报告和数据系统(BIRADS)分级对乳腺实性病变良恶性的诊断价值。方法:对158例(202个乳腺肿块)乳腺肿块患者采用UE检查,测量病变组织与周围组织弹性应变率比值,并根据UE结果对超声BI-RADS分级进行校正,与病理结果进行对照,构建ROC曲线。对比分析校正后BI-RADS分级法与BI-RADS分级法在诊断乳腺实性病变良恶性的敏感性、特异性、准确性、阳性预测值、阴性预测值。结果:202个病灶中,良性159个,恶性43个。校正后BI-RADS分级法对乳腺实性病变良恶性诊断的敏感性、特异性、准确性、阳性预测值、阴性预测值均明显高于BI-RADS分级法(P<0.05)。校正后BI-RADS分级法构建的ROC曲线与BI-RADS分级法及改良5分法、应变率比值法比较差异均有统计学意义(P﹤0.05)。结论:在乳腺实性病变良恶性鉴别诊断中UE、BI-RADS分级联合应用可提高诊断准确性,为临床进一步选择治疗方案有重要的指导价值。
Objective:To investigate the diagnosis value of improved elasticity scores method and strain ratio method combined with BI-RADS in the differential diagnosis of malignant and benign breast tumours.Methods:One hundred and fifty-eight patients with 202 breast lesions were accepted ultrasonic elastography examination,then calculate the strain ratio between the lesion and the nomal tissue.The BI-RADS was used for classification while UE was uses to adjust the results.The curve of ROC was employed to evaluate the combined use on malignant and benign breast lesions.The sensitivity,specificity,accuracy rate,positive predictive value and negative predictive value were compared before and after the adjustment of BI-RADS classification in the diagnosis of benign and malignant breast lesions.Results:202 breast lesions were confirmed by pathology,159 were benign and 43 were malignant.The sensitivity,specificity,accuracy rate,positive predictive value and negative predictive value of corrected BI-RADS were significantly higher than those of BI-RADS in the diagnosis of benign and malignant breast lesions(P〈0.05).There was significant difference between ROC curve of corrected BIRADS,improved 5-point method,strain rate method and original BI-RADS(P ﹤ 0.05).Conclusion:UE and BI-RADS classification combines ues has a high accuracy in the differential diagnosis of malignant and benign breast lesions.
