电生理监测在颅内动脉瘤夹闭术中的应用及临床观察 被引量:2

摘要 目的:对颅内动脉瘤夹闭术中电生理监测的应用效果进行临床观察。方法:随机性选取某院自2014年6月份以来进行手术治疗的颅内动脉瘤患者70例实施本次医学探究,根据患者手术中是否使用电生理监测分为治疗组和对照组,每组患者正好均分为35例。治疗组患者在进行手术治疗时使用神经电生理联合监测,对照组则不使用,对比两组患者手术后的并发症发生率、GOS评分、死亡率等指标进行比较。结果:通过比较发现在治疗组患者中,共有6例患者的体感诱发电位、运动诱发电位、脑干听觉诱发电位以及脑电图等没有出现显著变化,而且波幅明显的降低部分消失,通过进行干预,共有1例患者患有偏瘫,2例患者患有轻度偏瘫。分别在手术前、后对两组患者的脑血流量图、脑血容量图以及通过的平均时间进行比较,结果显示手术前两组患者各项指标无显著意义,但术后治疗组患者的脑血流量图与容量图均显著低于对照组患者(P<0.05);对两组患者的住院天数、GOS评分等指标进行比较,治疗组明显优于对照组(P<0.05)。结论:通过应用电生理监测在颅动脉瘤夹闭术中,能够及时的对患者的脑组织血流情况进行监测,并根据相应变化及时制定干预措施,有效降低了该类疾病的死亡率、致残率,提供了患者的预后。 Objective:To observe the effect of electrophysiological monitoring in intracranial aneurysm clipping.Methods:70 cases of intracranial aneurysm patients who had undergone surgical treatment since June 2014 were randomly selected,who were divided into treatment group and control group according to whether they were treated with electrophysiological monitoring,with 35 cases in each group.The treatment group was treated with neurophysiological cohort in surgery while the control group was not.The incidence of complications,GOS score and mortality were compared between the two groups.Results:By comparison,there were no significant changes in somatosensory evoked potentials,motor evoked potentials,brainstem auditory evoked potentials,and electroencephalogram(EEG)in 6 patients in the treatment group,and a significant reduction in amplitude or even was partially eliminated.A total of 1 patient hemiplegia,2 patients with mild hemiplegia.The cerebral blood flow chart,cerebral blood volume chart and the mean time of the two groups were compared before and after operation.The results showed that there was no significant difference between the two groups before operation,but the the blood flow chart and the volume chart of the treatment group were signigicant lower than thos of the control group(P〈0.05).The hospital days,GOS score and other indicators of the treatment group were significantly better than those in the control group(P〈0.05).Conclusion:Application of electrophysiological monitoring in the cranial aneurysm clipping surgery can timely monitor the blood flow of the patients’ brain tissue,which effectively reduces the mortality rate of such diseases,morbidity,and provides prognosis for the patients.
