超声去腐和注射式充填对邻接点以下龋损的疗效观察 被引量:3

摘要 目的:对比不同临床去腐及充填方法对邻接点以下龋损的疗效。方法:纳入62个患牙随机分为3组。A组(n=21)车针去腐+普通树脂充填、B组(n=20)车针结合超声去腐+普通树脂充填、C组(n=21)车针结合超声去腐+流动树脂结合普通树脂充填。记录各组的临床操作时间、并发症发生率、充填后即刻边缘密合性,评价术后3、6、12个月修复体边缘着色、牙龈炎症、牙髓反应等指标,对数据进行统计分析。结果:车针结合超声去腐与单纯车针去腐所用操作时间分别为(5.2±1.9)min、(3.7±1.2)min(P<0.05),邻牙损伤分别有0例和2例。流动树脂结合普通树脂充填与普通树脂充填所用时间分别为(3.1±0.8)min、(4.6±1.8)min(P<0.05),前者充填后边缘密合性优于后者(P<0.05)。术后3、6、12个月牙髓反应A、C组均发生;3组修复体边缘着色均随时间而增加,而牙龈炎症无明显变化。结论:邻接点以下龋损的治疗中,超声去腐可作为车针去腐方法的补充。注射式流动树脂结合普通树脂充填优于单纯普通树脂充填。 AIM: To observe the effects of different clinical operation techniques on the dental caries of the adjacent surface.METHODS: 62 teeth with adjacent caries were randomly divided into 3 groups.In group A(n=21) caries was removed by burs and the defects were filled with manual technique.In group B(n=20), caries was removed by burs combined with ultrasonic equipment, the defects were filled with manual technique.In group C(n=21), caries was removed by burs combined with ultrasonic equipment, and the defects were filled with injection and manual techniques.Operation time, operative complication rate, edge tightness of immediately after filling, Ryge''s clinical evaluation index of edge coloring, gingival inflammation and pulp reaction were evaluated 3, 6 and 12 months after filling.One-way ANOVA (alpha=0.05) and Chi-square test were used for the statistic analysis.RESULTS: The operation time of the combination caries removal group and the single method of bur removal was (5.2±1.9)min and (3.7±1.2)min (P〈0.05).The adjacent teeth injury was happened in 0 and 2 cases of the 2 groups respectively.The operation time of the filling with injection technique combined manual technique and the filling with manual technique was (3.1±0.8)min and (4.6±1.8)min (P〈0.05), respectively.The edge tightness immediately after filling in combined technique group was better than that of single manual technique(P〈0.05).After treatment the pulp reaction in the early 3 months was observed in 1 case of group A and C respectively.Edge coloring increased with time in the 3 groups.No change of gingivitis indexes was found in the 3 groups.CONCLUSION: In the treatment of adjacent surface caries, ultrasonic caries removal method can be used as a supplementary method to the routine bur removal.Filling with injection technique combined manual technique is better than filling with single manual technique.
