摘要
目的探讨以教师为标准化病人(Standardized Patients,SP)在神经内科系统体格检查教学的应用。方法在南方医科大学第二临床医学院神经内科实习生临床教学中先对8名教师随机抽取4人培训成为SP,再把32名实习生随机分成观察组和对照组,每组16名学生(每个教师带教4个学生)。观察组采用教师作为标准化病人,教师需讲授体格检查要点并模拟标准化病人让学生对其进行体格检查训练,并在训练过程中及时纠正学生的体格检查手法。对照组采用传统的以授课为基础的教学模式(Lecture-based Learning,LBL)并让学生两两随机配对互相体格检查。结果经授课后,观察组在体格检查考核成绩(笔试+操作)优于对照组。两组成绩经统计学检验,差异具有统计学意义(P<0.05)。问卷调查显示学生普遍欢迎以教师为标准化病人。结论以教师为SP的教学形式可在较短的时间内让实习生迅速掌握体格检查的各项细节,明显提高了神经内科实习生神经系统体格检查的教学质量。
Objective To explore the application of standardized patients(SP) with teachers in the teaching of physical examination in neurologydepartment. Methods In the clinical teaching of neurology intern of the Second Clinical Medical College of Southern Medical University, 4 out of 8 teachers were randomly selected and trained as SP. Then 32 interns were randomly divided into the observation group and the control group, each group has 16 students(each teacher takes 4 students). The observation group used teachers as standardized patients, teachers are required to teach physical examination points, simulate standardized patients, have them undergo physical examination and training, and correct their physical examination methods in the course of training. The control group adopted a traditional based teaching model(Lecture-based Learning, LBL) and asked students to randomly pair each other for physical examination. Results In the observation group after authorization, physical examination(written examination and operation) was better than the control group. The two groups showed a statistically significant difference(P〈0.05). Questionnaires show that students generally welcome teachers to be standardized patients.Conclusion The teaching mode in which teachers act as the SP could help interns to master the details of physical examinations quickly, and at the same time improve the teaching quality of neurological physical examination.
