摘要
目的探讨临床路径教学模式在综合医院儿科住院医师培训中的应用效果。方法将40名接受住院医师规范化培训的综合医院儿科住院医师随机分为临床路径组和传统带教组,各20名。临床路径组用以临床路径模式为基础的教学模式进行培训,传统带教组用传统的培训方法进行培训,均培训2年。培训结束后比较两组住院医师的理论知识水平、病例分析能力、临床操作能力和医患沟通能力。理论知识水平、病例分析能力采用笔试形式考察,临床操作能力考察住院医师儿科常用操作的熟练程度,医患沟通能力考察被患者家长投诉率。结果临床路径组、传统带教组住院医师培训结束理论知识考试得分分别为(76.22±3.34)、(75.78±2.61)分,两组相比P>0.05。临床路径组、传统带教组住院医师培训结束病例分析能力考试得分分别为(92.32±3.68)、(84.21±2.79)分,两组相比P<0.05。临床路径组、传统带教组住院医师培训结束临床操作考试得分分别为(88.23±3.62)、(83.18±3.01)分,两组相比P<0.05。临床路径组3名(15%)、传统带教组6名(30%)住院医师被患者家长投诉,两组被投诉率相比P<0.05。结论在综合医院儿科住院医师培训中采取临床路径模式的教学方法可以有效提高教学效果。
Objective To explore the application of clinical pathway teaching model in the pediatric residency training in the general hospital. Methods Forty pediatric residents were randomly divided into the experimental group and the con- trol group. The residents of the experimental group were trained by teaching model of clinical pathway, and the residents of the control group were trained by traditional methods. The theoretic knowledge level, case analysis ability, clinical skills, and physician-patient communication ability were assessed and compared after two-year training. Theoretic knowledge level and case analysis ability were assessed by written examination, the clinical skills were assessed by the proficiency of clinical practice, and physician-patient communication ability was assessed by complaint rate of parents. Results The theoretic knowledge scores of the experimental group and the control group were 76.22 ± 3.34 and 75.78 ± 2.61, respectively ( P 〉 0.05). Scores of case analysis ability of the two groups were 92.32 ± 3.68 and 84.21 ± 2.79 (P 〈 0.05 ). Scores of clini- cal skills were 88.23 ± 3.62 and 83.18 ± 3.01 (P 〈 0.05 ). More residents of the control group (30%) were complained by parents than that ( 15% ) of the experimental group (P 〈 0.05.). Conclusion The teaching model of clinical pathway in pediatric residency training can effectively improve the teaching effect.
