摘要
目的:建立本地区正常妊娠妇女甲状腺激素参考值,为探讨正常妊娠期妇女甲状腺功能提供参考依据。方法:选取2014年4月-2015年3月在本院进行正常产检的花都区正常妊娠期孕产妇为研究对象,按照妊娠时间分为孕早期组180例、孕中期组565例、孕晚期组143例,选取来本院健康体检中心进行健康体检的非妊娠期妇女213例作为对照组。采用化学发光法检测四组血清中TSH、FT3及FT4水平,比较检测结果。结果:孕早、中、晚期组及对照组血清中FT3分别为(4.20±0.66)、(4.00±0.73)、(3.73±0.59)、(3.65±0.64)pmol/L,其中孕早期组FT3水平明显高于孕晚期组和对照组(P<0.05),孕中期组FT3水平明显高于对照组(P<0.05)。四组FT4分别为(15.83±2.86)、(13.81±2.29)、(12.92±1.98)、(12.67±1.86)pmol/L,其中孕早期组FT4水平明显高于孕中、晚期组和对照组(P<0.05),孕中期组FT4水平明显高于对照组(P<0.05)。四组TSH分别为(1.69±1.18)、(1.80±1.02)、(2.05±1.07)、(1.93±1.06)m U/L,其中孕早期组TSH水平明显高于孕晚期组和对照组(P<0.05)。结论:本研究确定本地区正常孕妇不同孕期甲状腺激素水平变化的参考范围,有助于临床合理诊断妊娠期妇女甲状腺疾病。
Objective: To establish a normal reference value of thyroid hormones for normal pregnant women in our area, to provide some references for thyroid function and clinical study of normal pregnant women. Method: From April 2014 to March 2015, normal pregnant women accepted obstetric examination in our hospital were selected as study object and divided into the early pregnancy group of 180 cases, the middle pregnancy group of 565 cases and the late pregnancy group of 143 case, 213 non-pregnant women accepted heahh examination in health examination center of our hospital at the same period were selected as the control group.The serum TSH, FT3 and FT4 levels of four groups were detected by chemiluminescence method and compared.Result: The serum FT3 of the early, middle and late pregnant group and the control group were respectively ( 4.20±0.66 ), ( 4.00 ± 0.73 ), ( 3.73 ± 0.59 ) and ( 3.65 ±0.64 ) pmol/L, the serum FT3 of the early pregnant group was obviously higher than that of the late pregnant group and the control group ( P〈0.05 ), the serum FT3 of the middle pregnant group was obviously higher than that of the control group ( P〈0.05 ) .The serum FT4 of four groups were respectively ( 15.83 ± 2.86 ), ( 13.81 ± 2.29 ), ( 12.92 ± 1.98 )and( 12.67± 1.86 ) pmol/L, the serum FT4 of the early pregnant group was obviously higher than that of the middle and late pregnant group and the control group ( P〈0.05 ), the serum FT4 of the middle pregnant group was obviously higher than that of the control group ( P〈0.05 ) .The serum FSH of four groups were respectively ( 1.69± 1.18 ), ( 1.80 ± 1.02 ), ( 2.05± 1.07 ) and ( 1.93 ± 1.06 ) mU/L, the serum TSH of the early pregnant group was obviously higher than that of the late pregnant group and the control group ( P〈0.05 ) .Conclusion: This study ascertains the reference range of thyroid hormone of normal pregnant women in different gestational period in our area, which is helpful to clinically reasonably diagnose thyroid disease of pregnant women.
