摘要
目的探讨缺血性卒中患者胱抑素C(cystatin C,Cys C)与脑动脉粥样硬化性狭窄的关系。方法应用荷兰Philips公司生产的FD-20 C型臂血管造影系统,对299例缺血性卒中患者进行全脑血管造影及血浆Cys C检测,根据Cys C检测水平,分为对照组(Cys C正常)139例患者与异常组(Cys C异常)160例患者,比较其全脑血管造影。结果正常组(对照组)与异常组(病例组)脑动脉狭窄程度差异有统计学意义(χ2=16.94,P<0.05);异常组中颅内外动脉中重度狭窄和闭塞的比率显著升高,且以无侧支循环代偿(73.13%)及弥漫性病变(65.63%)为主;随着Cys C的升高,脑动脉狭窄程度逐渐加重。结论 Cys C是缺血性脑卒中患者脑动脉粥样硬化性狭窄的一种危险因素,脑血管狭窄与Cys C升高有关,Cys C在一定程度上反映脑血管狭窄程度。
Objective To explore the relationship between the cystatin C and cerebral atherosclerotic stenosis in patients with cerebral ischemic stroke. Methods For all 299 cases of ischemic cerebrovascular patients,cerebral angiography was performed by FD-20 C-arm angiography system which produced by the Dutch company Philips,and serum Cys C was detected. According to the levels of Cys C,all patients were divided into normal Cys C group( control group,n = 139) and high Cys C group( case group,n = 160),and then analyze the results of cerebral angiography between two groups. Results The differences of cerebral artery stenosis between the control group and case group were statistically significant( χ2=16. 94,P〈 0. 05). There were more cases of moderate-severe cerebral artery stenosis and occlusion in the case group than the control group. The majority of vascular involvement in the case group was diffuse without collateral circulation( 65. 63%and 73. 13%). With the increase of Cys C,the cerebral artery stenosis aggravated gradually. Conclusion Cys C may be a risk factor for cerebral artery stenosis in ischemic cerebrovascular disease,cerebral vascular stenosis is related with Cys C,which can reflect the severity of cerebral artery stenosis to some extent.
