血液透析联合血液灌流对终末期肾病患者ApoC3、血脂代谢及心脏结构与功能的影响 被引量:17

Effect of hemodialysis combined with blood perfusion on Apo C3,blood lipid metabolism,heart structure and function in patients with end-stage renal disease

摘要 目的:探讨血液透析联合血液灌流对终末期肾病(ESRD)患者Apo C3、血脂代谢及心脏结构与功能的影响。方法:80例ESRD患者随机分为对照组和观察组,每组40例。对照组患者给予单一血液透析治疗,观察组采用血液透析联合血液灌流治疗,比较2组患者治疗前、后Apo C3、血脂代谢及心脏结构与功能的变化。结果:治疗后观察组Apo C3水平显著下降[(184.98±54.90)μg/m L vs(161.57±41.48)μg/m L,P<0.05],对照组无明显变化[(187.92±50.31)μg/m L vs(177.92±50.31)μg/m L](P>0.05);观察组治疗后TC、TG和LDL-C显著下降(P<0.05),HDL-C显著增加(P<0.05);对照组治疗前、后各项血脂水平均无统计学差异(P>0.05);观察组患者治疗后血浆IL-6、AngⅡ及BNP水平改善程度均明显优于对照组(P<0.05);治疗后,2组患者心脏LVDd、LVSTd、LVPW和EF水平均显著改善(P<0.05),且观察组治疗后心脏结构功能更优(P<0.05)。结论:血液透析联合血液灌流能有效降低ESRD患者Apo C3水平,纠正患者血脂代谢紊乱,改善心肌重构和心功能,是治疗ESRD的重要方法之一。 Objective: To investigate the effects of hemodialysis combined with blood perfusion on Apo C3,blood lipid metabolism and cardiac structure and function in patients with end-stage renal disease( ESRD). Methods: Eighty patients with ESRD were randomly divided into observation group and control group,40 cases in each group. The control group was given a single hemodialysis treatment,and the observation group was treated with hemodialysis combined with blood perfusion. Apo C3,blood lipid metabolism and cardiac structure and function changes were compared between two groups before and after treatment. Results: After treatment,the Apo C3 levels in the observation group [( 184. 98 ± 54. 90) μg/m L vs( 161. 57 ± 41. 48) μg/m L]were significantly decreased( P 〈 0. 05),and the control group [( 187. 92 ± 50. 31) μg/m L vs( 177. 92 ± 50. 31) μg/m L]showed no significant change( P 〉 0. 05). TC,TG and LDL-C were significantly decreased after treatment in the observation group( P 〈 0. 05),and the HDL-C was significantly increased( P 〈 0. 05). There was no significant change in blood lipid levels before and after treatment in the control group( P 〉 0. 05). After treatment,the improvement of plasma Ang,IL-6 II and BNP in the observation group was significantly better than that in the control group( P 〈 0. 05). After treatment,the cardiac LVDd,LVSTd,LVPW and EF levels in two groups were significantly improved( P〈 0. 05),and the cardiac structure function in the observation group was better( P 〈 0. 05). Conclusion: Hemodialysis combined with blood perfusion can effectively reduce the level of Apo C3 in patients with ESRD,correct the disorder of blood lipid metabolism,improve myocardial remodeling and cardiac function,and is one of the important methods in the treatment of ESRD.
