摘要
目的 观察妊娠期亚临床甲减(甲状腺功能减退)以及甲状腺激素干预治疗对妊娠结局的影响.方法 选择亚临床甲减孕妇123例,按治疗方法不同分为两组,46例接受甲状腺激素治疗的亚临床甲减孕妇作为治疗组,77例未接受治疗的亚临床甲减孕妇作为观察组.100例正常孕妇作为对照组.三组分别在孕早期8~12周、孕16~20周及孕晚期28~32周检测血清促甲状腺激素(TSH)、游离甲状腺激素(FT4)、甲状腺过氧化酶抗体 (TPOAb)浓度.结果 2 780例定期产检孕妇中发生亚临床甲减123例,发生率为4.42%.正常妊娠对照组、观察组和治疗组糖代谢异常的发生率分别为7.0%、20.77%和10.86%;妊娠期高血压的发生率分别为2.0%、15.58%和2.17%;贫血的发生率分别为3.0%、18.18%和4.34%;自然流产的发生率分别为3.0%、25.77%和8.69%;围生儿死亡率分别为0、7.79%和4.34%;早产率分别为2.0%、15.58%和6.52%;统计学分析显示上述指标观察组显著高于对照组和治疗组(P<0.05),对照组与治疗组比较差异未见统计学意义(P>0.05).三组新生儿体质量比较差异未见统计学意义(P>0.05).结论 亚临床甲减孕妇不良妊娠结局发生率较高,甲状腺激素的干扰治疗可以降低上述并发症的发生率.
Objective To observe the effect of subclinical hypothyroidism and thyroid hormone therapy on pregnancy outcome.Methods A total of 123 pregnancy women with subclinic hypothyroidism were enrolled in this study. All the pregnancy women were divided into two groups: levothyroxine intervention group(n=46) received thyroid hormone therapy, and observation group(n=77) did not receive treatment.100 cases of normal pregnancy women were selected as a normal population control group. During pregnancy 8-12 weeks, 16-20 weeks,and 28-32 weeks, thy thyroid tests include FT4,TSH,TPOAb.Results In the total 2 780 cases,123 cases were subclinic hypothyroidism with a rate of 4.42%. In control group, observation group, and intervention group, the incidence of abnomal glucose metabolism was 7.0%, 20.77% and 10.86%;the incidence of hypertension was 2.0%, 15.58% and 2.17%; the incidence of anemia was 3.0%,18.18% and 4.34%; the incidence of spontaneous abortion was 3.0%, 25.77% and 8.69%; the incidence of newly born mortality was 0, 7.79% and 4.34%;the incidence of premature was 2.0%, 15.58% and 6.52%. Statical analysis showed that these indicators in observation group was significantly higher than that in the control and intervention group(P〈0.05). No significant difference was found between the control and intervention group(P〉0.05). There was no significant difference in the incidence of the weight of newborns among the three groups(P〉0.05).Conclusions Pregnant women with subclinical hypothyroidism have high incidence of adverse pregnancy outcomes,and those clinical application can be intervented by thyroid hormone treatment.
