眶内壁骨折高浓度血小板血浆的疗效观察

Observation on the effect of high concentration platelet-rich plasma for orbital wall fracture

摘要 目的观察高浓度血小板血浆(platelet—rich plasma,PRP)在眶内壁骨折可吸收材料内固定术应用的临床效果。方法眶内壁骨折60例(60眼),随机分为两组:临床试验组,30例,鼻内镜下瑞士可吸收材料联合PRP贴敷治疗;对照组,30例,单纯采用瑞士可吸收材料内固定治疗。术后随访1年,观察比较两组治疗效果。结果试验组术后消肿时间、眼球正常转动恢复时间均较对照组短(P=0.004,0.004)。试验组仅有1例发生内眦角轻度水平向回退,垂直向未见回退;对照组有7例发生内眦角轻度水平向回退,垂直向未见回退。两组患者术后均无感染,骨折愈合好,眼球无凹陷,双侧眼外观对称,眼球转动自如。结论PRP可适用于眼内壁骨折的修复和重建。 Objective To observe the clinical effect of application of high concentration platelet-rich plasma (PRP) in internal fixation by absorbable material for the treatment of orbital wall fracture. Methods Sixty eyes of 60 cases with orbital wall fracture were divided into two groups randomly. In the clinical trial group, 30 cases were treated with internal fixation by Swiss absorbable material combined with PRP applicator treatment under nasal endoscopy ; while in the control group , 30 cases were treated with internal fixation by Swiss absorbable material only. All patients were followed up for 1 years, and the therapeutic effects were observed and compared between the two groups. Results The time of detumescenee and the recovery time of eye movement in experimental group were shorter than those in control group ( P = 0. 004, 0.004). The mild level of canthus to the back occurred in 1 case of the trial group, no vertical regression. In the control group inner canthus mild horizontal vertical back occurred in 7 cases. No postoperative infection happened. All cases bone fracture healed, eyeball rotated freely without sag and symmetrical in appearance. Conclusion PRP is appropriate for the repair and reconstruction of orbital wall fracture.
