摘要
目的探讨细菌性肺炎新生儿体内T淋巴细胞亚群、NK细胞及其亚群和细胞因子白细胞介素-4(IL-4)和干扰素-γ(IFN-γ)的变化情况。方法选择医院新生儿科2014年1月-2015年12月住院细菌性肺炎新生儿40例为细菌性肺炎组和住院单纯性高胆红素血症新生儿40例为对照组;流式细胞术检测外周血CD_3^+、CD_4^+、CD8+、CD19+、NK细胞、CD3-CD56bright CD16neg/dim、CD3-CD56dimCD16bright、CD3-CD56neg CD16bright及IL-4和IFN-γ。结果细菌性肺炎组新生儿CD_3^+、CD_4^+和CD8+(57.97±3.24、34.82±6.52、21.85±2.82)均低于对照组(60.48±4.12、40.46±5.57、23.38±2.47)(P<0.05),细菌性肺炎组新生儿CD19+(33.18±6.67)高于对照组(24.47±2.29)(P<0.05);细菌性肺炎组新生儿NK细胞、CD3-CD56bright CD16neg/dim、CD3-CD56dimCD16bright、CD3-CD56neg CD16bright和对照组比较差异无统计学意义;细菌性肺炎新生儿IL-4水平(2.26±0.24)高于对照组(2.08±0.29)(P<0.05),细菌性肺炎新生儿IFN-γ水平和对照组比较差异无统计学意义。结论细菌性肺炎新生儿存在免疫功能紊乱,体内CD_3^+、CD_4^+和CD8+降低,CD19+、NK细胞、CD3-CD56bright CD16neg/dim、CD3-CD56dim CD16bright、CD3-CD56negCD16bright升高,IL-4升高。
OBJECTIVE To investigate the changes of lymphocyte subsets,NK cells and their subgroups as well as the cytokines interleukin-4(IL-4)and interferon-γ(IFN-γ)in neonates with bacterial pneumonia.METHODS Totally 40 neonates with bacterial pneumonia who were hospitalized the neonatology department from Jan 2014 to Dec2015were chosen as the bacterial pneumonia group,and 40 neonates with simple hyperbilirubinemia were set as the control group.The levels of peripheral blood CD3^+,CD4^+,CD8^+,CD19^+,NK cells,CD3-CD56 ^bright CD16^neg/dim,CD3-CD56^dimCD16^ bright,CD3-CD56^ neg CD16^bright,IL-4,and IFN-γwere detected by using flow cytometry.RESULTS The levels of CD3^+,CD4^+,and CD8^+of the bacterial pneumonia group were respectively(57.97±3.2),(34.82±6.52),and(21.85±2.82),lower than(60.48±4.12),(40.46±5.57),and(23.38±2.47)of the control group(P〈0.05).The CD19+level of the bacterial pneumonia group was(33.18±6.67),higher than(24.47±2.29)of the control group(P〈0.05).There was no significant difference in the level of NK cells,CD3-CD56 ^bright CD16^neg/dim,CD3-CD56^dimCD16^ bright,or CD3-CD56^negCD16^ bright between the bacterial pneumonia group and the control group.The IL-4level of the bacterial pneumonia group was(2.26±0.24),higher than(2.08±0.29)of the control group(P〈0.05);there was no significant difference in the IFN-γlevel between the bacterial pneumonia group and the control group.CONCLUSION The neonates with bacterial pneumonia have the immune dysfunction,the levels of CD3^+,CD4^+,and CD8^+are reduced,while the levels of CD19^+,NK cells,CD3-CD56 ^bright CD16^neg/dim,CD3-CD56^dimCD16^ bright,CD3-CD56^negCD16 ^bright,and IL-4are elevated.
