摘要
目的:研究磨牙区拔牙后不翻瓣即刻种植的短期临床效果。方法:磨牙微创拔除不翻瓣即刻植入钛种植体42枚(实验组),磨牙区骨量充足者常规延期植入钛种植体22枚(对照组),观察种植后两组种植体愈合情况,分别于术后当天、6个月时拍摄X线片,比较两组植体颈部近远中向的垂直骨吸收量;对两组患者进行相关满意度调查,统计手术耐受度、术后不良反应、修复体的咀嚼功能、美学感受、发音、舒适度6个因素满意度>8.5分者占调查总人数的百分比。结果:两组植体均获得良好的骨整合,无植体松动脱落等情况,实验组与对照组的垂直骨吸收量近似,差异无统计学意义(P>0.05);两组患者总体满意度均较高,各因素满意度>8.5分的患者占比均在80%以上。结论:磨牙区不翻瓣即刻种植短期内可获得较满意的临床效果。
Objective： To investigate the clinical efficacy of immediate implant placement after flap- less surgery in posterior extraction sockets. Method： 42 titanium implants were placed into the flapless sites immediately （experiment group）, 22 titanium implants were placed into the molar sites without bone defect （control group） ; observing dental implant healing condition of both groups. Comparing X- ray images on operation day and 6 months later of vertical bone absorption of mesio-distally direction of implant neck part of both groups; carrying out satisfaction survey of both groups, collecting data con- eerning operation tolerance level, post-operation adverse reaction, prosthesis masticatory function, aes- thetic feeling, pronunciation, comfort level, and the percentage of people with ＂ 〉 8.5＂ scores. Re- suits： All implants kept excellent osseointegration, no implants were failed. Vertical bone absorption of both groups were similar, differences were not statistically significant （ P 〉 0.05 ） ; both groups showed relatively high satisfaction level, number of patients with ＂ 〉 8.5＂ scores accounted for 80% of total patients. Conclusion： Immediate implant placement after flapless surgery in molar sites could yield satisfactory clinical results in short term.
