摘要
目的研究客家人群中肥胖性高血压患者的血尿酸水平与RAS系统、胰岛素抵抗的相关性。方法以本院2015年1月~2016年6月间收治的120例客家高血压患者作为研究对象,根据体质量(BMI)指数将患者划分为肥胖组与普通组;对比两组患者的RAS系统组分指标、胰岛素抵抗指标以及血尿酸水平;根据血尿酸表达水平将肥胖组患者分为高表达组、中表达组与低表达组,对比两组患者的RAS系统组分指标、胰岛素抵抗指标;分析血尿酸表达水平与RAS系统组分指标、胰岛素抵抗指数的相关性。结果肥胖组患者的血压、BMI及腰臀比均高于普通组(P<0.05)。肥胖组血尿酸、PRA、AGT、AngⅡ、HOMA-IR均高于普通组(P<0.01)。肥胖组中高表达组、中表达组与低表达组三组间PRA、AGT、AngⅡ、HOMA-IR均可见差异(P<0.05)。血尿酸水平与PRA、AGT、AngⅡ、HOMA-IR均可见正相关性,0<r<1,(P<0.05)。结论客家人群肥胖性高血压患者的血尿酸水平、RAS系统组分指标、胰岛素抵抗指数均明显高于非肥胖性高血压患者,在肥胖性高血压患者中,血尿酸水平越高RAS系统组分指标、胰岛素抵抗指数也随之升高,血尿酸水平与RAS系统组分指标、胰岛素抵抗指数间均具有明确的相关性。
Objective To study the correlation of serum uric acid level and RAS system and insulin resistance of patients with obesity hypertension in Hakka people. Methods 120 patients with hypertension in Hakka who were admitted to our hospital from January 2015 to June 2016 were selected as research objects. According to body mass index (BMI),patients were divided into the obesity group and the normal group.RAS system component index,insulin resistance index and serum urine acid level of patients in two groups were compared.According to expression level serum urine acid,obese patients were divided into the high expression group,moderate expression group and low expression group.RAS system component index and insulin resistance index of patients in two groups were compared. Correlation of expression level of serum uric acid,RAS system component index and insulin resistance index was analyzed. Results Blood pressure,BMI and waist-hip ratio of patients in the obesity group were all higher than those of the normal group (P 〈 0.05).Serum uric acid level,PRA,AGT,Ang Ⅱ ,HOMA-IR of the obesity group were all higher than those of the normal group(P 〈 0.01).Serum uric acid level,PRA,AGT,Ang Ⅱ ,HOMA-IR of high expression group,moderate expression group and low expression group in the obesity group all had differences (P 〈 0.05).Serum uric acid level was positively related to PRA,AGT,Ang Ⅱ ,HOMA-IR,0 〈 r 〈 1,P 〈 0.05. Conclusion Serum uric acid level,RAS system component index and insulin resistance index of obese patients with hypertension in Hakka were all significantly higher than those of non- obese patients with hypertension. Among obese patients with hypertension, patients with a higher serum uric acid level have higher RAS system component index and insulin resistance index. Serum uric acid level has respectively accurate correlation with RAS system component index and insulin resistance index.
