摘要 目的:评价引导组织再生术(GTR)在错畸形伴牙周病患者治疗中的应用价值。方法:共纳入68例患者,随机分为对照组和实验组,每组34例。对照组:进行正畸和牙周病治疗;实验组:治疗的同时应用GTR(Bio-Oss骨胶原联合Bio-Gide胶原膜),对比治疗后6个月患者的正畸疗效,治疗前、治疗后1个月和6个月的牙周指标包括探诊深度(PD)、附着丧失(AL)、牙龈退缩(GR)和骨密度值。结果:实验组的正畸疗效明显优于对照组,差异有统计学意义(P<0.05);治疗后1个月,两组PD、AL和GR值均较前降低,骨密度值升高(P<0.05),对照组和实验组比较无显著性差异(P>0.05);治疗后6个月对照组上述指标与治疗后1个月比较无显著性差异(P>0.05),实验组PD、AL和GR值进一步降低,骨密度值升高(P<0.05),两组比较差异明显(P<0.05)。结论:GTR可进一步改善错畸形伴牙周病患者正畸治疗和牙周修复的近期效果。 Objective To evaluate the value of guided tissue regeneration (GTR) applied in orthodontic treatment of patients with malocclusion and periodontal disease. Methods Sixty-eight patients were randomly divided into control group and experimental group, with 34 patients in each group. The control group was regularly treated, while the experimental group with regular treatment conbined with GTR (Bio-Oss collagen combined with Bio-Gide collagen membrane). Probing depth (PD), attachment loss (AL), gingival recession (GR), and bone mineral density (BMD) in both group were measured before treatment, 1 month after treatment and 6 month after treatment. Results The orthodontic effect in the experimental group was significantly better than the control group, and the difference was statistically significant (P〈0.05). There was no significant difference between the control group and the experimental group (P〉0.05). One month after treatment, the levels of PD, AL and GR in both group were decreased and the bone mineral density were increased (P〈0.05), and there was no significant difference between the control group and the experimental group (P〉0.05). Six month after treatment, the above indexes in the control group had no significant difference with those one month after treatment in the control group (P〉0.05), the values of PD, AL and GR in the experimental group were further decreased and the bone mineral density were further increased (P〈0.05), and there was significant difference between the control group and the experimental group (P〈0.05). Conclusion GTR can further improve the short-term effect of orthodontic treatment and periodontal repairment in patients with malocclusion and periodontal disease.
