摘要
目的 探讨甲状腺疾病患者血清中5种甲状腺激素水平的变化及其临床意义.方法 选择2015年8月至2017年2月我院收治的甲状腺疾病患者137例作为研究对象,检测患者血清中5种甲状腺激素水平,包括:促甲状腺素(TSH)、总三碘甲状腺原氨酸(T3)、总甲状腺素(T4)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4),并从甲亢和甲低两方面比较5项指标的阳性率.结果 对于甲亢的诊断,其阳性率高低分别为TSH(51.09%)〉FT3(18.25%)〉FT4(14.60%)〉T3(9.49%)〉T4(6.57%),TSH诊断甲亢的阳性率高于其他指标,差异有统计学意义(P〈0.05);对于甲低的诊断,其阳性率高低分别为TSH(39.42%)〉FT4(34.31%)〉FT3(14.60%)〉T4(7.30%)〉T3(4.38%),TSH诊断甲低的阳性率高于其他指标,差异有统计学意义(P〈0.05).结论 在甲状腺疾病诊疗中,对于患者血清进行5种甲状腺激素测定,可以提高诊断效果,其中TSH检测阳性率最高,对甲状腺疾病的诊断有重要指导意义.
Objective To investigate the changes and clinical significances of the levels of five types of serum thyroid hormone in patients with thyroid disease. Methods From August 2015 to February 2017, 137 cases of patients with thyroid disease in our hospital were selected as research objects. Five types of serum thyroid hormone were detected, including: thyroid stimulating hormone (TSH), three iodine thyroid original glycine (T3), total thyroxine (T4), free three iodine thyroid original glycine (FF3) and free thyroxine (FT4), compared the positive rates of five indicators in the diagnosis of hyperthyroidism and hypothyroidism. Results In the diagnosis of hyperthyroidism, the positive rates of five serum thyroid hormone were TSH (51.09%) 〉 FT3 (18.25%) 〉 FF4 (14.60%) 〉 T3 (9.49%) 〉 T4 (6.57%), the positive rate of TSH was higher than other indicators in the diagnosis of hyperthyroidism, the difference was statistically significant (P〈0.05) ; In the diagnosis of hypothyroidism, the positive rates of five serum thyroid hormone were TSH (39.42%) 〉FT4 (34.31%) 〉 FT3 (14.60%) 〉 T4 (7.30%) 〉 T3 (4.38%), the positive rate of TSH was higher than other indicators in the diagnosis of hypothyroidism, the difference was statistically significant (P〈0.05). Conclusion In the diagnosis and treatment of thyroid disease, the determination of five types of serum thyroid hormone can improve the diagnostic effect, TSH is the most sensitive indicator, which has important significance in the diagnosis of thyroid disease.
