砷类化合物致炎性损伤与细胞因子变化的Meta分析

A Meta-analysis of arsenic-induce inflammatory injury and alteration in cytokines

摘要 目的目前关于砷与炎性损伤的研究结论存在争议,应用Meta分析方法明确二者的关系,为揭示砷毒性作用机制提供参考。方法对中国知网、维普、万方、Cochrane、Pub Med、Springer、Web of Science等数据库进行文献检索,采用Meta分析对近年来国内外发表的实验资料进行分析,以标准化均数差(standard mean difference,SMD)描述组间差别。结果砷干预组IL-6[SMD为2.80,95%CI(1.87,3.72)]、IL-8[SMD为2.72,95%CI(1.63,3.82)]、IL-12[SMD为0.96,95%CI(0.04,1.87)]、NF-κB[SMD为2.76,95%CI(0.36,5.17)]和TNF-α[SMD为2.89,95%CI(2.03,3.75)]均高于对照组(P<0.05),IL-10[SMD为-1.13,95%CI(-1.80,-0.46)]和Viability[SMD为-5.27,95%CI(-8.52,-2.02)]均低于对照组(P<0.05)。亚组分析可见,高剂量(>5μmol/L或>5 mg/kg)砷暴露组的IL-2、IL-6、IL-8、NF-κB和TNF-α均高于对照组(P<0.05),Viability低于对照组(P=0.01);时间砷暴露(>24 h)组的IL-1、IL-6、IL-8、IL-12、NF-κB和TNF-α均高于对照组(P<0.05),IL-4、IL-10和Viability均低于对照组(P<0.05)。结论砷可以诱发炎性损伤,且高剂量、长时间的砷干预炎性损伤明显。 Objective There is controversy over the conclusion on the relationship between arsenic and inflammatory injury.Herein,a meta-analysis was performed to investigate the effects of arsenic exposure on inflammatory injury,which provides reference for revealing the mechanism of arsenic toxicity. Methods Literature was searched by independently searching databases,including the China National Knowledge Infrastructure,Vip,Wanfang,Cochrane,Pub Med,Springer and Web of Science. And a meta-analysis was used to retrieve experimental data on arsenic and inflammation published both in China and worldwide in the past few years. The differences between groups were described by standard mean difference( SMD). Result Levels of IL-6[SMD = 2. 80,95% CI( 1. 87,3. 72) ],IL-8[SMD =2. 72,95% CI( 1. 63,3. 82) ],IL-12[SMD = 0. 96,95% CI( 0. 04,1. 87) ],NF-κB[SMD = 2. 76,95% CI( 0. 36,5. 17) ] and TNF-α[SMD = 2. 89,95% CI( 2. 03,3. 75) ]in the arsenic exposed group was higher than that of the control group( P〈0. 05),while IL-10[SMD=-1. 13,95% CI(-1. 80,-0. 46) ] and viability[SMD =-5. 27,95% CI(-8. 52,-2. 02) ] was lower than control group( P〈0. 05). Subgroup analysis demonstrated that high doses( 〉5 μmol/L or〉 5 mg/kg) of arsenic exposure increase the expression of IL-2,IL-6,IL-8,NF-κB and TNF-α( P〈0. 05),and reduce the expression of Viability( P〈0. 05). Arsenic exposure times of 〉24 h was found to promote the production of IL-1,IL-6,IL-8,IL-12,NF-κB and TNF-α( P〈0. 05),suppress the production of IL-4,IL-10 and viability( P〈0. 05). Conclusion Arsenic can induce inflammatory injury,especially long exposure to high arsenic doses.
