输卵管脓肿、积液的CT、MRI诊断 被引量:7

摘要 目的分析输卵管脓肿、积液的CT及MRI影像学表现,与病理结果对照,以提高诊断准确率及鉴别诊断能力。方法回顾性分析26例经手术病理证实的输卵管脓肿、积液的CT、MRI表现,CT检查20例,全部病例均行平扫及增强扫描,MRI扫描6例,4例平扫,2例增强扫描。分析病变的形态特征、密度(信号)、强化特点、周围结构的变化情况,并与手术病理结果相对照。结果 26例中双侧输卵管脓肿、积液5例,右侧输卵管脓肿、积液12例,左侧9例;22例可见弯曲"腊肠样"表现,占84.6%(22/26);20例管壁轻度均匀性增厚,占76.9%(20/26),增强后动脉期及门脉期均表现为明显均匀强化;15例管腔内见分隔影,分隔明显均匀强化。结论输卵管脓肿、积液的特征性CT、MRI表现为"腊肠样",增强后壁及分隔呈均匀较明显强化,具有较高的诊断价值。 Objective To explore the CT and MRI findings in diagnosis of the hydrosalpinx/pyosalpinx with pathologic findings.Methods 26 cases confirmed by pathology were included in this study, 20 patients underwent CT non-contrast and contrast-enhanced scanning, 4 cases underwent MRI non-contrast scan and the others underwent contrast-enhanced scaning.Imaging features of hydrosalpinx/pyosalpinx lesions were studied retrospectively, with depiction of lesions including shape, density, and intensified pattern.CT and MRI findings were compared with surgical findings and pathology.Results In 26 cases, there were 5 hydrosalpinx/pyosalpinx in bilateral tube, right tube was 12 lesions,and left tube was 9 lesions, the tube sign of the botuliform were shown in 22 lesions (84.6%), 77%(20/26) of which manifested wall with apparent enhancement,incomplete septa could be seen at the inside wall of the 15 cases, and the wall and septa demonstrated contrast enhancement distinctly.Conclusion The tube sign of the botuliform is special findings to hydrosalpinx/pyosalpinx with apparent enhancement on CT and MRI, which may have important diagnostic value.
