摘要
目的探讨CAD结合3D打印技术在全踝关节置换(TAR)中的应用价值。方法将广州军区广州总医院2010年1月至2015年1月收治的28例踝关节炎手术患者,随机分为观察组(14例)和对照组(14例),观察组在CAD结合3D打印个性化导板技术辅助下行TAR,对照组采取常规TAR。观察两组手术时间、术中出血量、住院时间及住院费用,比较两组术后Kofoed评分差异,记录术后并发症发生情况。结果患者获随访12~36个月,平均随访时间27.3个月。两组手术时间、术中出血量、住院时间及住院费用并无明显差异(P>0.05);术后1个月、1年Kofoed评分观察组高于对照组(P<0.05);观察组术后并发症发生率较对照组低,但差异无统计学意义(P>0.05)。结论 CAD结合3D打印技术辅助TAR能显著提高手术精度,改善长期疗效。
Objective To explore the value of computer assisted design (CAD) combined with 3D printing in total ankle replacement (TAR). Methods A total of 28 patients with ankle arthritis who were treated surgically in Guangzhou General Hospital from January 2010 to January 2015 were randomly divided into the observation group (n = 14) and the control group (n = 14), the observation group was performed TAR assisted by CAD combined with 3D printed personalized guide template, while patients in the control group underwent routine TAR. Operation time, estimate intraoperative blood loss, hospital stay and hospital costs were observed in two groups, postoperative Kofoed scores were compared between two groups, and complications were also recorded. Results Patients were followed up for 12 to 36 months, with an average of 27.3 months. There were no statistical differences of operation time, estimate intxaoperative blood loss, hospital stay and hospital costs between two groups; Kofoed scores at one months, one year postoperatively in observation group were higher than those in control group (P 〈0.05); The incidence of postoperative complications in observation group was lower than that in control group, but there is no statistical difference (P 〉0.05). Conclusion TAR assisted by CAD combined with 3D printing could significantly improve the accuracy of the surgery and enhance the long-term efficacy.
