摘要
目的了解上海市浦东新区远郊社区卫生服务中心全科医生职业倦怠与离职意愿现状,分析引起职业倦怠的原因,并找出对策。方法使用职业倦怠调查问卷、工作满意度问卷及离职意愿问卷对浦东新区远郊6家社区卫生服务中心85位全科医生进行问卷调查,对其结果进行数据分析。结果浦东远郊社区全科医生职业倦怠情况严重,零倦怠3人,检出率3.53%;轻度倦怠48人,检出率56.47%;中度倦怠25人,检出率29.41%;重度倦怠9人,检出率10.59%。其中情感耗竭维度得分为(20.28±6.76)分,检出率为24.71%;人格解体维度得分为(10.45±6.45)分,检出率为37.65%;成就感维度得分为(24.42±7.31)分,检出率84.71%。工作满意度处于中等水平,其中工作回报满意度得分为(4.44±1.99)分;工作本身满意度得分为(16.65±4.51)分;领导管理满意度得分为(20.78±5.68)分;人际关系满意度得分为(11.44±2.62)分。离职意愿得分为(7.74±3.40)分,得分未超过总分的50%;通过职业倦怠与工作满意度、离职意愿的相关性分析得出情感耗竭与工作本身和离职意愿具有相关性,相关系数为0.511;人格解体与工作本身、领导管理、人际关系具有相关性,相关系数分别为-0.239、-0.255、-0.435;成就感与工作本身、领导管理、人际关系、离职意愿具有相关性,相关系数分别为0.325、0.396、0.388、-0.038。从工作满意度与离职意愿的相关性分析得出离职意愿与工作本身、领导管理、人际关系呈负相关,相关系数分别为-0.453、-0.219、-0.269。结论上海浦东远郊全科医生职业倦怠检出率高,离职意愿不是很高,工作满意度处于中等水平,管理部门应及时找出原因并采取合理措施来关爱远郊社区全科医生,逐步降低职业倦怠,稳定全科医生队伍。
[Objective]To know about the status of job burnout and turnover intention among general practitioners in suburb CHSC(Community Health Service Center) of Pudong District of Shanghai,analyze the reasons of job burnout and find its solutions.[Methods]By using the job burnout questionnaire,job satisfaction questionnaire and turnover intention questionnaire,85 general practitioners in 6 CHSC were investigated,and the result data were analyzed.[Results]The job burnout status among general practitioner in suburb CHSC was severe. Only 3 persons had no job burnout with the detection rate of 3.53%. There were48 persons with mild burnout,25 persons with moderate burnout and 9 persons with severe burnout,which the detection rate was56.47%,29.41% and 10.59%,respectively. The score of emotional discomfort dimension was(20.28 ±6.76) points,with the detection rate of 24.71%,the score of personality disintegration dimension was(10.45 ±6.45) points,with the detection rate of37.65%,and the score of achievement dimension was(24.42±7.31)points,with the detection rate of 84.71%. Job satisfaction was at a moderate level. The score of job reward satisfaction,job itself satisfaction,leadership management satisfaction and interpersonal relationship satisfaction was(4.44 ±1.99) points,(16.65 ±4.51) points,(20.78 ±5.68) points and(11.44 ±2.62)points,respectively. The score of turnover intention was(7.74±3.40)points,which did not exceed 50% of the total score. Through the correlation analysis on job b urnout,job satisfaction and turnover intention,it was concluded that there was a correlation between emotional exhaustion,job itself and turnover intention,with the correlation coefficient of 0.511,the personality disintegration was correlated with work itself,leadership management and interpersonal relationship,which the correlation coefficient was respectively-0.239,-0.255 and-0.435,while the achievement sense was correlated with work itself,leadership management,interpersonal relationship and turnover intention,which the correlation coefficient was respectively 0.325,0.396,0.388and-0.038. Through the correlation analysis on job satisfaction and turnover intention,it was concluded that turnover intention was negatively correlated with work itself,leadership management and interpersonal relationship,which the correlation coefficient was respectively-0.453,-0.219 and-0.269.[Conclusion]The detection rate of job burnout among general practitioners in Pudong suburb of Shanghai is high,the turnover intention is not very high,and the job satisfaction is at the middle level. The management department should identify the causes and take reasonable measures to protect general practitioners in Pudong suburb of Shanghai,gradually reduce the job burnout,and insure the stability of community health service team.
