摘要
目的:探讨创伤患者血清胱抑素C(cystatin C,Cys C)、脑钠肽(brain natriuretic peptide,BNP)水平及其与创伤严重程度的相关性。方法:收集就诊于我院急诊科的120例急诊创伤患者。根据创伤严重程度评分(injury severity score,ISS),将120例患者分为轻伤(41例)、重伤(43例)、危重伤(36例)3组。分别于入院后0小时、24小时、3天和7天检测3组患者血清Cys C、BNP水平。分析3组患者血清Cys C、BNP水平与创伤严重程度的相关性。结果:3组患者0小时血清Cys C、BNP水平均无明显差异(P>0.05);轻伤组0小时、24小时、3天和7天血清Cys C、BNP水平均无明显差异(P>0.05);重伤组24小时、3天和7天血清Cys C、BNP水平均分别高于0小时(P<0.05),3天和7天血清Cys C水平均分别高于24小时(P<0.05);危重伤组24小时、3天和7天血清Cys C、BNP水平均高于0小时(P<0.05),3天和7天血清Cys C、BNP水平均分别高于24小时(P<0.05),7天血清Cys C水平高于3天(P<0.05);分别与轻伤组24小时、3天和7天相比,重伤组和危重伤组同时间血清Cys C、BNP水平均升高(P<0.05)。Pearson相关分析显示,血清Cys C水平与ISS评分之间呈正相关(r=0.912,P<0.01)。血清BNP水平与ISS评分之间呈正相关(r=0.896,P<0.01)。结论:不同程度的创伤患者不同时间有不同程度的血清Cys C、BNP水平的升高。Pearson相关分析显示:血清Cys C、BNP水平与创伤严重程度呈正相关,提示血清Cys C、BNP水平是创伤严重程度的重要指标,可以为临床评估创伤严重程度提供重要的参考价值。
Objective:To observe the levels of serum cystatin C(Cys C),brain natriuretic peptide(BNP)in traumatic patients and correlation analysis with traumatic severity.Methods:A total of 120 emergency traumatic patients in emergency department of our hospital were rolled from December 2015 to December 2016,Who were divided into minor trauma group(n=41),severe trauma group(n=43)and critical trauma group(n=36)according to the injury severity score(ISS).The levels of serum Cys C,BNP of the patients in the 3groups were detected on 0hours,24 hours,3days and 7days after admission respectively.Pearson correlation analysis of the levels of serum Cys C,BNP and ISS.Results:There were no significant differences in the levels of serum Cys C,BNP on 0hours between the three groups(P〈0.05);There were no significant differences in the levels of serum Cys C,BNP on 0hours,24 hours,3days and 7days in minor trauma group(P〈0.05);The levels of serum Cys C,BNP on 24 hours,3days and 7days were all higher than those of 0hours in severe trauma group(P〈0.05),and the levels of serum Cys C on 3days and 7days were both higher than those of 24hours(P〈0.05);The levels of serum Cys C,BNP on 24 hours,3days and 7days were all higher than those of 0hours in critical trauma group(P〈0.05),the levels of serum Cys C,BNP on 3days and 7days were both higher than those of 24hours(P〈0.05),and the levels of serum Cys C on 7days were higher than those of 3days(P〈0.05);The levels of serum Cys C,BNP in severe trauma and critical trauma groups were significantly higher compared with minor trauma group on 24 hours,3days and 7days(P〈0.05).Pearson correlation analysis,the level of serum Cys C were positively correlated with ISS(r=0.912,P〈0.001),the level of serum BNP were positively correlated with ISS(r=0.896,P〈0.001).Conclusion:Different levels of traumatic patients had different levels of serum Cys C,BNP increased at different times.Pearson correlation analysis showed that the levels of serum Cys C,BNP were both positively correlated with traumatic severity,which suggested that the levels of serum Cys C,BNP may be important indicators of traumatic severity and could provide important reference value for clinical evaluation of traumatic severity.
