摘要
目的探讨白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)及基质金属蛋白酶-13(MMP-13)在骨性关节炎中的表达及相关性。方法采用实时荧光定量PCR(RT-PCR)法及酶联免疫吸附(ELSIA)法检测72例骨性关节炎患者及32例正常对照者关节液中IL-1β、TNF-α及MMP-13的表达,对骨性关节炎患者病情严重程度进行美国纽约特种外科医院(HSS)评分,采用Pearson相关分析法分析骨性关节炎患者IL-1β、TNF-α与MMP-13的相关性以及它们与HSS评分之间的相关性。结果骨性关节炎患者关节液中IL-1β、TNF-α、MMP-13基因及蛋白表达水平均高于正常对照者(P<0.01),骨性关节炎患者关节液中IL-1β、TNF-α、MMP-13蛋白水平均与骨性关节炎严重程度呈正相关,IL-1β和TNF-α均与MMP-13蛋白水平呈正相关。结论 IL-1β、TNF-α及MMP-13与骨性关节炎发生、发展有密切关系,并且与病情严重程度相关,关节液中MMP-13的高表达可能是通过IL-1β及TNF-α等炎性细胞因子激活、上调。测定骨性关节炎患者关节液中IL-1β、TNF-α及MMP-13水平对骨性关节炎的诊断、病情判断和治疗具有重要意义。
Objective To investigate the expression of interlukin - 1β ( IL - 1β), tumor necrosis factor - α (TNF- α) and matrix metalloproteinase- β (MMP- β) in the synovial fluid of osteoarthritis (OA) patients and their correlations with OA. Methods The expression of IL - 1β, TNF - α and MMP - β in the synovial fluid of 72 OA pa- tients and 32 normal persons were assessed by RT - PCR and ELSIA. The severity of OA was accessed by the Hospital for Special Surgery (HSS) score. The pairwise correlations between IL - 1β, TNF - α or MMP - β and HSS score were ana- lyzed by Pearson correlation analysis. Results The mRNA and protein levels of IL - 1β, TNF - α and MMP - β of OA patients were all significantly higher than those of normal persons ( P 〈 0.01 ). There were significant positive correlations of IL - 113, TNF - a and MMP - β with the severity of OA. The levels of IL - 1β and TNF - α had significantly positive correlation with MMP - β in OA patients. Conclusion IL - 1β, TNF - α and MMP - β are closely related to the development and progression of OA, as well as correlated to the severity of OA. The high expression of MMP - 13 in the synovial fluid may be activated by IL - 1β and TNF - α. It is important to measure the levels of IL - 1β, TNF - α and MMP - β in the synovial fluid for the diagnosis, evaluation and treatment of osteoarthritis.
