摘要
目的调查我国三级甲等医院急诊医务人员锐器伤发生的现状,为临床治疗提供参考依据。方法采用自行设计的网络问卷,对全国29个省、市、自治区共114家三级甲等医院急诊医务人员2015年3-5月至2016年3-5月内锐器伤发生的情况进行调查。结果 1 212名调查对象中,383人在过去12个月内发生过锐器伤,平均发生密度为0.88次(人·年);不同职业、年龄、地区医务人员锐器伤的发生率差异具有统计学意义(P<0.05);安瓿瓶、头皮钢针、注射器针头、采血针是造成医务人员锐器伤的主要器具,发生率分别为51.2%、49.3%、33.7%、25.3%;发生锐器伤后,94.3%的医务人员能够正确做好即时处理措施;锐器伤上报率为2.8%;发生锐器伤的医务人员中有25.8%的人暴露前未注射过乙肝疫苗。结论应该加强急诊护士、年轻急诊医务人员、中部急诊医务人员的管理及培训,加大危险环节的监督及管理;提高安全器具的普及率;健全职业暴露上报体系。
OBJECTIVE To investigate the current situation of sharp injury among emergency medical staff in tertiary hospitals of China,so as to provide references for clinical treatment.METHODS The self-designed questionnaire was used to survey the prevalence of sharp injuries among emergency medical staff from 114 tertiary hospitals in 29 provinces,municipalities and autonomous regions from Mar.to May 2015 to Mar.to May 2016.RESULTS Among 1212 subjects,a total of 383(31.6%)subjects had sharp injuries in the past 12 months,and the average density was 0.88times(per person,per year).There were significant differences in the incidence of sharp injuries among emergency staff with different occupations,ages,and areas(P〈0.05).Ampoules,scalp steel needles,syringe needles,and blood collection needles were the main equipments causing the sharp injuries,and the incidences were 51.2%,49.3%,33.7%、25.3%,respectively.After sharp injury,94.3% of medical staff correctly completed the real-time processing measures.The sharp injury reporting rate was 2.8%.Among the injured staff,25.8% of the staff had not been vaccinated against hepatitis B before exposure.CONCLUSION National and relevant hospital management departments should strengthen the management and training of emergency nurses,young emergency medical staff,and emergency medical staff in the central region,increase the supervision and management of dangerous links,improve the penetration rate of safe equipment,and improve the occupational exposure reporting system.
