孤立性肺结节实施多层螺旋CT灌注成像的鉴别诊断效果分析 被引量:11

摘要 目的研究孤立性肺结节实施多层螺旋CT灌注成像的鉴别诊断效果。方法抽选2013年10月—2015年10月在该院接受治疗的孤立性肺结节患者60例,经病理证实良性、恶性及炎性各20例。所有患者均接受多层螺旋CT检查,以各项相关指标为标准对3组患者的灌注成像结果进行对比分析。结果良性的孤立性肺结节通透性(PS)、通过时间(MTT)、血容量(BV)及血流量(BF)分别为(4.6±3.1)m L/(min·100 g)、(6.3±1.5)s、(3.6±1.6)m L/100 g、(21.8±10.4)m L/(min·100 g)与炎性、恶性相比,差异有统计学意义(P<0.05),而3组在对比剂通过时间上分别为(6.3±1.5),(7.0±0.6),(6.6±1.8)s,则差异无统计学意义(P>0.05)。结论孤立性肺结节应用多层螺旋CT灌注成像技术能有效鉴别良性、恶性及炎症病灶,其具有优异的诊断学价值,所获得的影像学数据及评估可为后期治疗方案的科学制定提供精准的临床参考依据。 Objective To research the effect of multi-slice spiral CT perfusion imaging in differential diagnosis of solitary pulmonary nodule. Methods 60 cases of patients with solitary pulmonary nodules in our hospital from October 2013 to October 2015 were selected and the pathology showed that there were 20 cases of malignant patients, 20 cases of benign patients and 20 cases of inflammatory patients, and all patients received the multi-slice spiral CT examination, and the perfusion imaging results in each group were compared and analyzed according to various related indicators. Results The PS, MTT, BV and BF of the benign solitary pulmonary nodules were respectively (4.6±3.1)mL/(min·100 g),(6.3±1.5)s,(3.6±1.6)mL/100 g,(21.8±10.4)mL/(min·100 g), and the difference had statistical significance(P〈 0.05), and the contrast-medium pass time in each group was respectively (6.3±1.5),(7.0±0.6),(6.6±1.8)s, and the differences had no statistical significance(P〉0.05). Conclusion The application of multi-slice spiral CT perfusion imaging in differential diagnosis of solitary pulmonary nodule can effectively diagnose the benign, malignant and inflammatory lesions with excellent diagnosis value, and the acquired imaging data and evaluation can provide accurate reference for the scientific formulation of treatment plan in later stage.
