摘要 目的比较Pentacam眼前节分析仪和Topolyzer角膜地形图仪测量Kappa角的一致性。方法前瞻性对照研究。对2015年6-8月在解放军180医院眼科由同一检查者分别行飞秒全激光屈光手术的屈光不正患者29例(57只眼)采用Pentacam和Topolyzer直接测量Kappa角,并应用Topolyzer进行虹膜定位图像采集导入EX500准分子激光机虹膜定位切削模式生成Kappa角。对三种方法得出的Kappa角(水平X值和垂直Y值)应用组内相关系数和Bland-Altman分析方法进行一致性评价。结果组内相关系数分析表明三组数据一致性好。Bland-Altman分析方法X值95%LoA1和2组-0.14-0.09;1和3组-0.15-0.12;2和3组-0.10-0.11。Y值95%LoA1和2组-0.14-0.03;1和3组-0.17-0.08;2和3组-0.10-0.11。结论三种测量方法结果一致性好,可相互替代、校正和补充。Pentacam眼前节分析仪和Topolyzer角膜地形图仪两种仪器均可获得准确的Kappa角数据。 Objective To compare the Kappa Angle consistency measuring using Pentacam front section analyzer and Topolyzer corneal topography instrument. Methods Prospective controlled study. All 29 patients with refractive error (57 eyes) from the Department of Ophthalmology in Quanzhou #180 Hospital were received femtosecond laser refractive surgery from June 2015 to Au- gust 2015 were measured Kappa Angle using Pentacam and Topolyzer, and applied Topolyzer iris lo- cation model to acquire image and imported EX500 excimer laser iris location cutting pattern to gen- erate Kappa Angle by the same inspectors respectively. The consistency was evaluated by using three methods of Kappa Angle (vertical level X and Y values) Intraclass correlation coefficient and Bland - Altman analysis method. Results Intraclass correlation coefficient analysis of three sets of X and Y values was uniform. X 95% LoA1 of group 2 was-0.14 - 0.09; Group 1 and 3 was -0.15 - 0.12; Group 2 and group 3 was -0.10 - 0.11. Y value 95% LoA of group 1 and group 2 was-0.14 - 0.03; Group 1 and group 3 was-0.17 - 0.08; Group 2 and group 3 was -0.10 - 0.11 by Bland - Altman analysis method. Conclusions The three methods of measuring results are consistent, can substitute, correct and supplement each other. Pentacam front section analyzer and Topolyzer corneal topography can obtain accurate Kappa Angle data.
