摘要
目的探讨妊娠期亚临床甲状腺功能减退与甲状腺自身抗体水平的相关性。方法选择2014年1月-2015年12月在安吉县妇幼保健院产科门诊进行正规产检的妊娠期亚临床甲状腺功能减退孕妇96例作为观察组,正常孕妇96例作为对照组,测定两组血清甲状腺功能和甲状腺自身抗体。结果观察组血清促甲状腺激素(TSH)水平高于对照组(P<0.05),观察组血清游离甲状腺素(FT_4)水平和对照组比较,差异无统计学意义(P>0.05);观察组甲状腺过氧化物酶抗体(TPOAb)和甲状腺球蛋白抗体(TGAb)水平均高于对照组(P<0.05);观察组TPOAb和TGAb阳性率均高于对照组(P<0.05);观察组TPOAb和TGAb均和TSH呈正相关(P<0.05),TPOAb和FT_4呈负相关(P<0.05),TGAb和FT_4无显著相关性(P>0.05)。结论妊娠期甲状腺功能减退患者血清TPOAb和TGAb水平升高,血清TPOAb和TGAb水平和甲状腺功能有相关性。
Objective To explore the correlations between subclinical hypothyroidism and thyroid autoantibodies levels in pregnancy.Methods Ninety-six pregnant women with subclinical hypothyroidism receiving regular prenatal examination in Obstetric Outpatient of the hospital from January 2014 to December 2015 were selected as observation group,and ninety-six normal pregnant women were selected as control group. Thyroid function and serum thyroid autoantibodies in the two groups were determined. Results The level of serum thyroidstimulating hormone（ TSH） in observation group was statistically significantly higher than that in control group（ P〈0. 05）. There was no statistically significant difference in serum free thyroxine（ FT_4） between observation group and control group（ P〈0. 05）. The levels of thyroid peroxidase antibody（ TPOAb） and thyroglobulin antibody（ TGAb） in observation group were statistically significantly higher than those in control group（ P〉0. 05）. The positive rates of TPOAb and TGAb in observation group were statistically significantly higher than those in control group（ P〈0. 05）. In observation group,both TPOAb and TGAb were positively correlated with TSH（ P〈0. 05）,TPOAb was negatively correlated with FT_4（ P〈0. 05）,there was no significant correlation between TGAb and FT_4（ P〉0. 05）. Conclusion Serum levels of TPOAb and TGAb increase in patients with hypothyroidism in pregnancy,serum TPOAb and TGAb levels are correlated with thyroid function.
