下颌智齿位点行异种骨保存后骨壁厚度与吸收关系研究 被引量:1

摘要 目的:探讨一种脱蛋白猪骨矿物基质(DPBM,The Graft)与脱蛋白牛骨矿物基质(DBBM,Bio-Oss)在下颌智齿位点保存后颊舌侧骨壁厚度与垂直吸收、水平吸收相关性研究,为临床应用提供参考。方法:选取19例双侧下颌智齿中高位垂直阻生需拔除患者,左右两侧随机植入The Graft和Bio-Oss,双侧均覆盖可吸收胶原膜(Bio-Gide R)。分别于术后即刻(T0)、术后12周(T12)、术后24周(T24)时均拍摄锥形束CT,测量两组牙槽窝颊舌侧骨壁厚度及相应高度、宽度。利用SPSS21.0对双侧下颌智齿位点保存后(含两种异种骨)的牙槽窝颊舌侧骨壁初始厚度与相应高度、宽度改变值做相关性分析。结果:下颌双侧智齿拔除行位点保存术后24周,舌侧骨壁初始厚度(L)与其高度(HL)、宽度(WL)的改变值具有相关性;颊侧骨壁初始厚度(B)与相应高度(HB)改变值具有相关性;骨壁初始总厚度(Th)与总高度(H)、总宽度(W)的改变值具有相关性。进一步对上述指标进行线性回归分析发现,L与HL、WL改变值,B与HB改变值,Th与H改变值具有线性关系,直线回归方程分别为Y=-0.815X+0.073、Y=-0.762X+1.459,Y=-0.102X+0.546,Y=-0.145X+0.721。结论:下颌智齿拔除行位点保存24周内,牙槽窝骨壁初始厚度分别与其垂直向、水平向吸收呈负相关关系。 Objective: To discuss deproteinized porcine bone mineral (DPBM, The Graft ) and deproteinized bovine bone mineral (DBBM, Bio-Oss ) in extraction socket preservation of bilateral mandibular wisdom teeth, the relationship between alveolar bueeal or lingual bone thickness and thier vertical and horizontal absorbtion, providing reference for clinical application. Methods: 19 cases were selected in this study,whose occlusional plane of third molars required removal were moderately below or parallel to the second molar. The left and right sides were implanted The Graft and Bio-Oss randomly. All the alveolar sockets were covered with bioabsorbable collagen membrane (Bio-Gide ). After immediate surgery (T0), 12 weeks after surgery (T 12), 24 weeks after surgery(T24), patients were taken CBCT respectively. Measured the thickness, height, width of both sides of the buccal and lingual alveolar bone wall. Using SPSS21.0 software to make a correlation analysis between the thickness of buccal and lingual alveolar bone wall and relative height,width respectively. Results: 24 weeks after surgery, the correlation existed between the change amount of the lingual wall thickness (L) and corresponding initial heigt (HL), width (WL); the change amount of the buccal initial total bone wall thickness (B) and relative height(HB); the change amount of initial thickness of the bone of buccal and lingual side and the total width (W), the total height (H). Further linear regression analysis with these indicators, It was lbund that liner relationship existed between the change amonnt of L and HL, WL; the change amount of B andHB; the change amount of Th and H. The Linear regression equation was Y=-815X+0.073, Y=-762X+1.459, Y=-0.102X+0.546, Y=-0.145X+0.721. Conclusions: 24 weeks after mandibular wisdom teeth socket preservation, negative correlation exist in the alveolar wall initial thickness and its absorption of vertical and horizontal amount respectively.
