摘要
目的 检测深静脉血栓形成(DVT)患者血浆miR-146a及纤溶酶原激活物抑制因子1(PAI-1)的表达水平,探讨miR-146a调控PAI-1的可能机制及其对深静脉血栓形成患者的临床意义.方法 标本分为DVT病例组、有明确DVT危险因素但临床排除DVT的高危因素组和健康对照组各30例.抽取各组外周血血浆,从中提取miRNA,采用RT-qPCR法检测miR-146a的相对表达量.用ELISA法测定血浆中PAI-1的含量,分析miR-146a、PAI-1及各临床参数之间的关系.结果 ①DVT组的血浆miR-146a的相对表达量与健康对照组比较明显下降,差异有统计学意义(P〈0.01).高危因素组患者髋关节置换术(THA)后miR-146a的相对表达量与健康对照组比较亦有明显下降,妇科肿瘤患者miR-146a的相对表达量与健康对照组比较明显上升.②DVT组、高危因素组的PAI-1含量分别与健康对照组比较均有明显上升.③应用Pearson相关分析法分析,miR-146a与PAI-1呈负相关(r=-0.832,P〈0.01),D-二.聚体浓度与miR-146相对表达量呈负相关(r=-0.582,P〈0.05),而D-二聚体与PAI-1含量呈正相关(r=0.764,P〈0.01).结论 ①DVT患者miR-146a相对表达量下调,并分别与D-二聚体及PAI-1含量呈负相关,提示miR-146a可能在DVT的纤溶-抗纤溶系统失衡过程中发挥重要调节作用.②DVT及高危因素组患者PAI-1均有升高,提示纤溶抑制在血栓形成过程中的重要作用.
Objective To detect the expression level of miRNA-146a and plasma PAI-1 in peripheral blood of the DVT patients.To explore the clinical significance in patients with DVT.Methods The specimens were divided into 3 groups:DVT cases group,the patient with the highly DVT risk factor but exclude clinical diagnose of DVT group,healthy control group,each group include 30 cases,selected peripheral blood plasma of each cases.The extraction and purification of miRNA from the plasma,using the methods of RT-qPCR to detect the relative expression level of miR-146a.The expression of PAI-1 in serum was determined by enzyme-linked immunosorbent assay,analysis the relationship between miRNA146a,PAI-1 and various clinical parameters in each group.Results (1)The relative expression of plasma miR-146a in DVT group decreased significantly than healthy control group (P〈0.01).The content of miR-146a in the high risk factors group after THA in comparison with healthy control group also decreased significantly (P〈0.01),whilethe relative expression level of miR-146a in gynecological tumor in the high-risk group was compared with the healthy control group increased significantly.(2)The level of PAI-1 in DVT group and high risk factors groupwas significantly increased than the healthy control group (P〈0.01).(3)Using Pearson correlation analysis method to analysis the relationship between the relative expression of plasma miR-146 concentration and the plasma level of PAI-1,the plasma miR-146a was negatively correlated with PAI-1 (r=-0.832,P〈0.01).Conclusion (1)The relative expression of miR-146a in DVT group was downregulated,and the expression was negatively correlated with the content of D-dimmer and PAI-1,which show that the miR-146a plays an important modulation role in the pathogenesis of DVT.(2)The plasma level of PAI-1 both in DVT group and high-risk factor group were increased,but increased more significantly in DVT group.It commanded that the important role of fibrinolysis in the formation of thrombosis.
