摘要
目的 探讨2型糖尿病患者血清胱抑素C(Cys C)水平与原发性高血压发病的相关性.方法 选择577例正常尿蛋白且肾小球滤过率(eGFR)≥60 ml/(min·1.73 m2)的2型糖尿病患者,登记其一般资料后停用血管活性药物2周,并通过常规疗法使血糖达到控制标准,采集其晨起空腹静脉血,测量血清Cys C水平,将患者划分为四组:A组(261例)Cys C〈0.79 mg/L,B组(153例)Cys C 0.79~0.94 mg/L,C组(97例)Cys C 0.95~1.10 mg/L,D组(66例)Cys C〉1.10 mg/L.对比各组血压及高血压患病情况,分析血清Cys C水平与血压的相关性.结果 四组收缩压与舒张压均呈D组〉C组〉B组〉A组趋势,且两两对比差异均有统计学意义(P〈0.05);D组高血压总患病率及程度明显高于其他各组,差异有统计学意义(P〈0.05);血清Cys C水平与收缩压、舒张压均呈正相关(r=0.651,0.537;P〈0.05).结论 2型糖尿病患者血清Cys C水平越高,其血压水平越高,高血压发生率及程度越高.血清Cys C水平可能是2型糖尿病患者并发高血压的危险因素.
Objective To investigate the relationship between serum cystatin C (Cys C) and primary hypertension in patients with type 2 diabetes mellitus.Methods Chose five hundred and seventy-seven patients with type 2 diabetes mellitus whose uromicroprotein were normal and glomerular filtration rate (eGFR) were ≥60 ml/(min·1.73 m2).After their general information were recorded, vasoactive drugs were discontinued for 2 weeks, and blood glucose were controlled to standards by conventional therapy, then their morning fasting venous blood were collected to measure serum Cys C.Patients were divided into four groups by serum Cys C levels.Cys C in group A (261 cases) were 〈0.79 mg/L, in group B (153 cases) were 0.79-0.94 mg/L, in group C (97 cases) were 0.95-1.10 mg/L, in group D (66 cases) were 〉1.10 mg/L.Blood pressure and prevalence of hypertension in each group were compared.The relationship between serum Cys C and blood pressure were analyzed.Results Systolic blood pressure and diastolic blood pressure both showed the trend of group D 〉 group C 〉 group B〉 group A, the differences were significant in every two groups (P〈0.05).The total prevalence rate and grade of hypertension in group D were significantly higher than other groups, the differences were significant (P〈0.05).The serum Cys C level was positively correlated with systolic blood pressure and diastolic blood pressure (r=0.651, 0.537;P〈0.05).Conclusions With the serum Cys C increased, the blood pressure level in patients with type 2 diabetes increases, the incidence and degree of hypertension increases.Serum CysC level may be a risk factor for patients with type 2 diabetes mellitus complicated with hypertension.
