摘要
目的:对比研究X光片和多排螺旋CT对骨关节创伤的诊断价值。方法:回顾性分析我院2014年1月-2015年10月收治的124例骨关节创伤可疑骨折患者的病例资料,所有研究对象均行X光片和多排螺旋CT检查,对比两种检查方法在骨关节创伤诊断中的应用价值。结果:手术所见或病理学检查结果显示124例骨关节创伤可疑患者中有86例(共113处)发生骨折,有38例未发生骨折;86例骨折患者中X光片显示共有60例骨折,多排螺旋CT显示共有80例骨折;38例未发生骨折患者中X光片显示共有18例骨折,多排螺旋CT显示共有6例骨折;多排螺旋CT在骨关节创伤诊断中灵敏度、准确度和特异度均较X光片显著升高,而误诊率和漏诊率均较X光片显著降低,差异均具有统计学意义(P<0.05)。结论:多排螺旋CT在对骨关节创伤诊断中灵敏度、准确度和特异度均较X光片显著升高,而误诊率和漏诊率均显著降低,更具应用价值。
Objective: To compare and study the diagnostic values of bone and joint trauma with X ray pictures and multi slice spiral CT. Methods: The clinical datas of 124 cases of patients with suspected fracture of bone and joint trauma treated in our hospital form Jan 2014 to Oct 2015 were retrospectively analyzed,who were examined by X ray and multi slice spiral CT. The application values of the two kinds of examination methods in the diagnosis of bone and joint trauma. Results:The results of surgery or pathological examination showed that there were 86 cases( a total of 113 fractures) in the 124 cases of patients with bone joint trauma,other 38 cases without fracture. 86 cases of fracture patients with X ray showed a total of 60 cases of fractures,and the multi row spiral CT showed a total of 80 cases of fracture. 38 cases of non fracture patients with X ray showed a total of 18 cases of fracture,and the multi row spiral CT showed a total of 6 cases of fracture. The sensitivity,accuracy and specificity of multi slice spiral CT in the diagnosis of bone and joint trauma were significantly higher than those of the X ray,while the rates of misdiagnosis and missed diagnosis were significantly lower than those of the X ray,and the differences were statistically significant( P 〈 0. 05). Conclusion: The sensitivity,accuracy and specificity of multi slice spiral CT in the diagnosis of bone and joint trauma are significantly higher than those of the X ray,while the rates of misdiagnosis and missed diagnosis are significantly lower,which has more application value.
