摘要
目的探讨高血压脑出血(hypertensive cerebral hemorrhage,HICH)患者血清肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6与血管内皮生长因子(VEGF)的表达及临床意义。方法选择2013年5月—2016年5月西安市第九医院收治的HICH 108例作为观察组,同期在该院进行健康体检的健康体检者108例作为对照组。对观察组入院即刻及对照组健康体检时血清TNF-α、IL-6与VEGF水平进行比较,并对观察组入院即刻及入院后12 h、24 h、48 h、7 d血清TNF-α、IL-6与VEGF水平进行比较。结果观察组入院即刻血清TNF-α、IL-6及VEGF水平均明显高于对照组健康体检时,两组比较差异具有统计学意义(P<0.05)。观察组不同时间点血清TNF-α、IL-6与VEGF水平总体比较差异均具有统计学意义(P<0.05);观察组血清TNF-α、IL-6与VEGF水平在入院后48 h内不断上升,入院后48 h达峰值,入院后7 d明显降低,不同时间点间同一指标两两比较差异均具有统计学意义(P<0.05)。结论 HICH后,在一定时间内患者血清TNF-α、IL-6与VEGF水平均明显增高,急性期最为显著。血清TNF-α、IL-6及VEGF可作为HICH早期诊断的实验室指标。
Objective To investigate expressions and clinical significances of serum tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and vascular endothelial growth factor (VEGF) in patients with hypertensive cerebral hemorrhage.Methods A total of 108 patients with acute hypertensive cerebral hemorrhage admitted during May 2013 and May 2016 were selected as observation group, and other 108 healthy people taking medical examinations at the same period were selected as control group.Serum TNF-α, IL-6 and VEGF levels were compared between the time of admission in observation group and the time of taking medical examinations in control group.Serum TNF-α, IL-6 and VEGF levels were compared between the time of admission and 12 h, 24 h, 48 h and 7 d after admission in observation group.Results TNF-α, IL-6 and VEGF levels at the time of admission in observation group were significantly higher than those at the time of taking medical examinations in control group (P〈0.05).The differences in serum TNF-α, IL-6 and VEGF levels at different time points in observation group were statistically significant (P〈0.05).In observation group, TNF-α, IL-6 and VEGF levels were gradually increased within 48 h after admission, and peak values were found at 48 h after admission;the levels were significantly decreased at 7 d after admission;the differences in the same index between each two different time points were statistically significant (P〈0.05).Conclusion Serum TNF-α, IL-6 and VEGF levels can be found significantly increased in patients with hypertensive cerebral hemorrhage, and the increased levels are more significant in acute stage.Serum TNF-α, IL-6 and VEGF levels can be used as indicator for early diagnosis of hypertensive cerebral hemorrhage.
