经皮肺穿活检病理诊断在肺部特殊病原体感染中的应用价值 被引量:2

The Application Value of Percutaneous Aspiration Lung Biopsy with the Aid of Thoracic Imaging on Diagnosis of Pulmonary Infection Caused by Specific Pathogens

摘要 目的:探讨胸部影像学引导下经皮肺组织穿刺活检病理诊断在肺部特殊病原体感染中的应用价值。方法:回顾性分析2007年1月-2016年12月在福建医科大学附属闽东医院呼吸科住院,在超声或CT引导下经皮肺组织穿刺取得病理组织明确病原体确诊的肺部特殊病原体感染性疾病61例,收集全部病例临床资料,并对相关资料进行分析。结果:61例患者经皮肺穿活组织送病理检查58例确诊,3例未确诊。其中肺隐球菌病32例,肺结核9例,非结核分枝杆菌肺病2例,肺马尔尼菲青霉病1例,肺奴卡菌病1例,肺放线菌病4例,肺组织胞浆菌病1例,肺曲霉菌病3例,肺念珠菌病1例,肺毛霉菌病1例,肺孢子菌肺炎2例,肺粪类圆线虫病1例。诊断阳性率为95.08%,特异性为100%,并发症发生率为8.20%。结论:肺部特殊病原体感染诊断困难,难以明确病原体,胸部影像学(超声或CT)引导下经皮肺组织穿刺活检取得病理组织学检测联合特殊染色等检测手段可快速明确诊断,起到事半功倍的作用,值得临床推广应用。 Objective:To discuss the application value of percutaneous aspiration lung biopsy with the aid of thoracic imaging on diagnosis of pulmonary infection caused by specific pathogens.Method:A total of 61 cases of pulmonary infection caused by specific pathogens which were collected from inpatients of Respiration Department of Mindong Hospital affiliated to Fujian Medical University from January 2007 to December 2016 and diagnosed through percutaneous aspiration lung biopsy with the aid of ultrasonic or CT were reviewed and analyzed.Result:61 cases underwent biopsy by percutaneous lung biopsy,58 cases were diagnosed,and 3 cases were not diagnosed.Among those confirmed cases,32 cases were pulmonary cryptococcosis,9 cases were tuberculosis,2 cases were nontuberculous mycobacteria,1 case was pulmonary penicilliposis marneffei,1 case was pulmonary nocardiosis,4 cases were pulmonary actinomycosis,1 case was pulmonary histoplasmosis,3 cases were pulmonary aspergillosis,1 case was pulmonary candidiasis,1 case was pulmonary mucormycosis,2 cases were pneumocystis pneumonia,1 case was pulmonary strongyloidiasis.The diagnoses results pointed to a high positive rate of 95.08%,with specificity of 100% and a complication incidence rate of 8.20%.Conclusion:Although pulmonary infection caused by specific pathogens is usually difficult to diagnose,percutaneous aspiration lung biopsy with the aid of thoracic imaging and specific staining can quickly produce clear results,which deserves more clinical applications.
