摘要
目的总结胼胝体压部(SCC)可逆性孤立性病变的MRI、~1H-MRS特点,初步研究病灶持续存在时间与表观扩散系数(ADC)值的相关性。方法回顾性分析15例SCC可逆性孤立性病变患者的临床及影像表现,探讨SCC病灶持续时间与年龄、最大横截面积、ADC值的相关关系,以病灶持续时间为因变量(Y),年龄(X_1)、最大横截面积(X_2)、ADC值(X_3)为自变量,进行逐步回归分析,求出多元线性回归模型。结果病灶位于SCC中线区,单发,边界清楚,呈等/长T_1、长T_2信号,T_2-FLAIR及DWI序列呈高信号,ADC图低信号,增强后无强化,平均经过18.4天后消失。MRS均未见明显异常表现。ADC值与病灶持续时间呈中度正相关(r=0.624,P<0.05),病灶持续时间的影响因素包括最大横截面积和ADC值(R^2=0.618,F=2.924,P=0.012),多元逐步回归分析结果显示,ADC值对病灶持续时间有正向预测作用(R^2=0.389,F=8.277,P=0.013)。结论 SCC可逆性孤立性病变的MRI表现具有明显的特征性,对其明确诊断具有重要意义,并可根据其ADC值初步推测病灶持续存在的时间。
Objective To conclude the MRI and 1^H-MRS characteristics of the reversible and solitary lesion in Splenium of Corpus Callosum( SCC),and study the correlation between lesion duration and ADC value. Methods Clinical and imaging data of 15 cases were retrospectively analyzed; the correlation between the SCC lesion duration and the age,the area and the ADC value were all explored. Multiple regression analysis was then performed. The lesion duration as dependent variable( y),the age,the area and the ADC value were independent variables( X1,X2,X3). Finally,a multiple linear regression model was derived. Results 1. The solitary lesions located in median region of SCC had clear boundary,equal/long T1,long T2,hyperintensity on T2-FLAIR and DWI sequences and hypointensity in the corresponding ADC images,no enhancement. On average,the lesions disappeared after 18. 4 days. There were no obviously abnormal appearances in MRS images. 2. ADC values were positively correlated with disease duration( r = 0. 624,P 0. 05),factors affecting the duration of lesions including the largest cross-sectional area and the ADC value( R^2= 0. 618,F = 2. 924,P = 0. 012),multiple stepwise regression analysis showed that the ADC value has a positive forecast for the duration of the lesion( R^2= 0. 389,F= 8. 277,P = 0. 013). Conclusion The MRI features of the reversible isolated lesions of the corpus callosum have obvious characteristics which are of great significance for the diagnosis of the disease,and the existing time of the lesion can be estimated according to the ADC value.
