摘要
目的探讨急性肺栓塞合并下肢深静脉血栓介入治疗的临床效果。方法急性肺栓塞合并下肢深静脉血栓患者36例,按随机数字表法分为2组,对照组18例,行下腔静脉滤器置入后采取全身静脉溶栓,观察组18例,行下腔静脉滤器置入后采取导管袪栓联合导管溶栓,比较两组治疗效果及相关并发症发生情况。结果观察组术后血氧分压(82.34±2.57)mm Hg、血氧饱和度(98.52±2.87)%、休克指数(0.72±0.05),均优于对照组相应指标,差异有统计学意义(P<0.05)。观察组术后肢体周径差(2.62±0.68)cm、Miller指数(3.28±0.81)分、静脉通畅度评分(3.16±0.62)分,均优于对照组相应指标,差异有统计学意义(P<0.05),观察组总有效率94.44%,显著高于对照组的77.78%,差异有统计学意义(P<0.05),两组无严重不良反应发生。结论对急性肺栓塞合并下肢深静脉血栓患者采取介入治疗的疗效较好,且操作简便,安全性好。
Objective To investigate the clinical effect of interventional therapy for acute pulmonary embol-ism complicated with deep venous thrombosis of lower extremity. Methods 36 patients with acute pulmonary embol-ism complicated with deep venous thrombosis of lower extremity were randomly divided into 2 groups. The control group was given by intravenous thrombolysis treatment for inferior vena cava filter, and the observation group was un-derwent inferior vena cava filter placement after catheter thrombectomy combined with catheter thrombolysis. The inci-dence of related complications was compared. Results In the observation group, postoperative blood oxygen pressure was (82. 34 ± 2. 57) mmHg,oxygen saturation was (98. 52 ± 2. 87) %,shock index was (0. 72 ± 0. 05),which were better than the corresponding index of the control group ( P 〈 0. 05 ). The difference of limb circumference was (2. 62 ± 0. 68) cm,Miller index was (3. 28 ± 0. 81 ),and venous patency score was (3. 16 ± 0. 62) points in the observation group, which were better than the corresponding index of the control group ( P 〈 0. 05 ). In the obser-vation group, the total efficiency was 94. 44% , significantly higher than that in the control group (77. 78% ) (P 〈 0. 05 ) . The two groups had no serious adverse reactions occurred. Conclusion The interventional treatment of acute pulmonary embolism complicated with deep venous thrombosis of lower limb is better, and the operation is simple and the safety is high.
