摘要
目的:评价3D-SPACE序列在深部浸润型子宫内膜异位症(DIE)的应用价值。方法:连续收集妇产科医院41例经腹腔镜证实为子宫内膜异位症的病人入组,入组病人均进行前瞻性MRI扫描,包含MRI常规序列和3D-SPACE特殊序列。采用SPSS软件统计分析两名阅片者(R1、R2)对DIE的诊断性评价和一致性评价。结果:两名医师(R1、R2)对深部浸润性子宫内膜异位症各亚型的诊断灵敏度、特异度和正确率分别结果如下:右侧宫骶韧带(R1:0.923、1.000、0.976;R2:0.923、1.000、0.976,K值1.000),左侧宫骶韧带(R1:0.923、0.964、0.951;R2:1.000、0.964、0.976,K值0.945),阴道直肠膈(R1:0.917、1.000、0.951;R2:1.000、1.000、1.000,K值0.901),直乙状结肠(R1:0.929、0.889、0.902;R2:1.000、0.926、0.951,K值0.898),子宫直肠陷凹(POD)(R1:0.893、0.846、0.878;R2:0.929、0.846、0.902,K值0.945),膀胱和输尿管结果一致均为(R1:1.000、1.000、1.000;R2:1.000、1.000、1.000,K值1.000),且两名医生诊断结果一致性非常好。结论:MRI的3D-SPACE序列作为高分辨率MRI成像技术对深部浸润型子宫内膜异位症的诊断准确性高,建议3D-SPACE序列作为DIE的有效补充序列。
Purpose: To evaluate the value of MR 3D-SPACE sequence in assessment of deeply infiltrating endomelriosis (DIE). Methods: This prospective study included 41 women with clinical suspicion of DIE. The MR images were compared with laparoscopy and pathology findings. Sensitivity, specificity, and accuracy of MRI for diagnosis of DIE were assessed by two experienced readers (R1, R2) . Results: The sensitivity, specificity and accuracy of MRI in diagnose of DIE at each of the specific sites by two readers were as follows: RUSL (R1 : 0.923, 1.000, 0.976; R2: 0.923, 1.000, 0.976, K-value: 1.000) , LUSL (R1: 0.923, 0.964, 0.951; R2: 1.000, 0.964, 0.976, K-value: 0.945) , RVS (RI: 0.917, 1.000, 0.951; R2: 1.000, 1.000, 1.000, K-value: 0.901) , recto-sigmoid (RI: 0.929, 0.889, 0.902; R2: 1.000, 0.926, 0.951, K-value: 0.898) , pouch of douglas (POD) (R1: 0.893, 0.846, 0.878; R2: 0.929, 0.846, 0.902, K-value: 0.945) , bladder and ureters (R1: 1.000, 1.000, 1.000; R2: 1.000, 1.000, 1.000, K-value: 1.000) , and Kappa value was with excellent accordance between two readers. Conclusions: 3D-SPACE sequence demonstrates highly accuracy in diagnosing deeply infiltrating endometriosis, which is recommended as a powerful supplemental sequence in patients with DIE. Experienced doctors have higher diagnostic accuracy of deeply infiltrating endometriosis involving rectum and sigmoid.
